The Upper Canada Family Health Team is comprised of a group of health care professionals working collaboratively to provide comprehensive and accessible health care. The goals of the Upper Canada Family Health Team include preventing illness and promoting the health of patients. We aim to reduce the number of patients in Brockville who do not have a health care provider, reduce ER visits for minor illness, and reduce hospital admissions for chronic illnesses.

Mission: Our mission is to maintain and improve the health of our community by providing patient centred, comprehensive and responsive care of the acute and chronic physical, mental , and emotional needs of our patients and by providing a high level of health promotion and prevention services.

For the month of December our social committee has organized our team to participate in a reverse advent calendar for the local food bank! This initiative is happening in both our Brockville and Gananoque location.

If you feel unwell with any of these symptoms, the best thing to do is to stay home from work/school, self-isolate at home – including avoiding contact with other household members – and to get a COVID-19 test.

The Brockville Assessment Centre is not very busy right now. Don’t be afraid to come to the centre to get an appointment FOR TODAY if you meet the criteria (you are showing symptoms of COVID-19; have been contacted by public health or the COVID Alert app that you have been exposed to a confirmed case; are a resident or work in a setting that has a COVID-19 outbreak, as identified by your local public health unit; or as part of a targeted testing initiative group directed by the government) – The center is open until 5:30pm today.

Update: The Brockville Assessment Centre is transitioning to an online appointment booking system. It may take some time to get the appointment system running. In the meantime, individuals that meet the new COVID-19 Testing Eligibility Criteria (those with symptoms and close contacts of positive cases as informed by public health or the COVID Alert app) can self–present to the centre between operating hours to confirm eligibility for testing and to receive an appointment time. At this point there is no need to call ahead and we anticipate that individuals will be provided a time to come back that same day.

As directed by Ontario Health, access to COVID-19 testing is open to anyone, similar to how Emergency Rooms operate. We have been hearing a lot of concerns about people from outside LGL accessing the Assessment Centres, however, data from the Brockville Assessment Centre for last week shows that only about 15% of the visits were from outside of LGL. The increase in demand for testing is happening all across Ontario, and we are working vigorously towards solutions. Stay tuned. #WeAreInThisTogether
