We provide a free, in-home assessment, which will help the individual to express his or her own needs. Our Registered Nurse will assess and identify the areas that the individual requires assistance and support. She will then prepare a tailor-made home care action plan to ensure the delivery of proper personal care service and/or household management. Our goal is to monitor our clients very closely and to identify any changes in behavior or physical function so that the care is efficient and effective for the person. Doctor visits with #agingparents #medicalappointments Prep: Encourage your parents to let you be involved in their medical appointments and get consent to ‘sit in’ the appointment. Talk to your parents about anything that they are feeling or any changes that you or they notice beforehand and make notes. Bring a list of updated meds (pharmacy will print you a list).
During visit (using your notes): Go through all of the concerns. 1. Find out:
When was last time blood work was done?
2. What is your parents’ blood pressure and is it in the normal range?
3. Has there been a weight loss?
4. Don’t be shy to ask to see a specialist for any conditions, memory changes or symptoms.
Also:
1. Ask about side-effects and the safety of combining medication.
2. Ask follow up questions.
3. Ask for the doctor to explain terms or concepts you don’t understand.
4. Ask for the results of recent medical tests (sometimes doctors only bring up results if there is an abnormality).
5. Ask if your doctor is communicating with other members of your parent’s health care team – don’t assume they are. Be prepared to give them the contact name of physiotherapists, pharmacists, specialists and the other people who are involved in caring for your parent.
6. Check that your doctor is okay with taking the generic drug whenever possible (generic drugs are usually more affordable than brand names).

Have some baseline tests done including a memory test, medication review (also ask the pharmacist),bloodwork and urine check. The Home Care division of my company Gold Cross Home Care was acquired (sold) to Bayshore HealthCare Ltd. effective July 3, 2018. At this time, I would like to thank the team Bayshore HealthCare Ltd. (Hamilton, Ontario) for their excellence in transitioning and caring for my clients; my Admin team especially my daughter and partner Jacqueline DeRubeis who assisted me on all levels with the merger and acquisition. She is now the Care Manager for Bayshore Hamilton Private Division. I am grateful for my loyal staff for transitioning as true professionals. I would also like to thank all of my clients, staff, and business colleagues for their loyalty and support over the past 9 years. Pneumonia can develop easily especially in the elderly. Always have a working thermometer in your home and an elevated temperature is an indicator of infection. The average body temperature, taken with a thermometer in the mouth, is 37ºC (98.6ºF), but anywhere between 36.5ºC and 37.2ºC (97.7ºF and 99ºF) can be considered normal.
