Firstly, what is antalgia or antalgic posture?
▪️It describes the visible changes in how people hold or carry themselves when there is intense low back pain.
🔹Although it looks “bad”… it’s actually a protective mechanism. As with many protective mechanisms, it can be too extreme and postural habits may sometimes linger long after pain has subsided.
The goal with rehabilitation is to adapt. Get better.

Secondly, direction or intent is so so important to return to normal movements.
▪️1st VS 2nd image results of CORRECT directional mobilization VS INCORRECT directional mobilization.
▪️3rd image shows the directional PNF work to gain a more upright posture for daily activities.

Specificity, in this context, matters. Understanding the individual’s presentation far exceeds treating the diagnosis of a lumbar disc herniation. Not everyone benefits from McKenzie style low back extension, they can even make people worse. This patient required flexion and lateral flexion movements.

Understand the person, treat and prescribe accordingly.
▪️Exercises are specific to a person, not a diagnosis.
