Hi everyone. Be advised that the wellness component of our program that is offered by the Niagara Region is suspended until April 6th. However, the Ambulatory Rehab program is still treating patients and accepting new referrals based on regular communication between Public Health and Hotel Dieu Shaver Health and Rehabilitation Centre. Further updates will be provided as necessary in this ongoing situation.

Day 15: There’s no place like home! The South Niagara and Wellness Centre offers an outpatient program that promotes mobility, function and independence. An important and troubling effect of taking away a senior’s independence is how it can impact their health and overall well-being. Program benefits include increased functional independence at home. Patients set realistic treatment goals and to continue their therapy at home through self-management strategies. The program helps avoid readmission and empower patients and families to integrate into their community and manage chronic health conditions.
