Not every case is ideal. I'm not used to sharing my mistakes but one only learns from looking back on them and learning how to manage such setbacks.

This case presented with missing 24 and 25. Upon implant placement, I unfortunately placed 24 more buccal than intended, with an obvious full buccal wall loss. The management included Guided Bone Regeneration GBR using bone particulates covered with pericardium membrane. Finally 2 screw retained crowns were delivered regaining function and esthetics.

The patient is satisfied with the results but I learned that day that there are many considerations to take care of before attempting placement.

Challenging yourself is essential to grow. This case is not perfect, but I learned a lot deal from it than my other routine work. When I'm faced with a case like this again, yesterday's mistakes will be tomorrow's saves.

Immediate implant placement of upper central using vestibular approach due to presence of periapical lesion with labial fenestration. A direct SSA or temporary prosthesis would have made outcome better but that was not in my arsenal at the time.


After placement of the implant, I fabricated a temporary composite crown over orthodontic wire for esthetic purposes only for the period of 3 months until osseointegration occurred.

After that, I did a frenectomy and after it's healing added a soft tissue graft for optimal labial contour and preservation of the surrounding periodontium. Then a screw retained crown was placed for the patient. Special thanks to my mentor Ahmad Mano

Many dentists think overdentures is an outdated treatment option. Although its use has been limited with the advancement in implant dentistry and the famous "All-On-X" concept, it is still the suitable and sometimes an ideal option for patients who actually need it.

I believe in "patient-centered treatment planning" because every patient comes with a collection of different situations. Treatment planning is based on data collection first and foremost, then choosing an option that best suits THAT patient, not a standard plan for all patients regardless of their condition and inputs present.

This patient had atrophic jaws with diminished support to the lips and cheeks. Regenerative dentistry with augmentation would be difficult from a biologic, financial, and timely perspective. Graftless solutions such as the PATZI protocol using zygomatic implants would necessitate a surgery a patient was not ready for.

The ideal situation from all perspectives for the patient was to go for Implant-supported overdenture which gives the patient the strong retention she wishes for while at the same time limiting the surgeries and cost to a minimum and still maintaining proper and easy hygiene measures.
