For years, pain was explained with a fairly tidy equation. Something hurts, so something must be damaged, so find it and fix it.

It makes sense. It's also incomplete.

Sometimes tissues are involved. Sometimes a nerve is affected. And sometimes the nervous system has become more protective, producing very real pain even when ongoing damage doesn't explain it. Sometimes it's more than one of these at once.

That changes the question. Instead of only asking "What have I damaged?", it's worth asking "What is driving my pain?"

Which one have you been asking?

"We can't find anything wrong."

If you've heard that sentence, you'll know it isn't always the relief it sounds like.

The scan was reassuring. The blood tests were normal. You've tried treatment after treatment. And yet you still hurt.

This month I'm unpicking something I've become increasingly interested in: why pain and damage aren't the same thing, and why there isn't just one type of pain. There are three.

Knowing which one is driving things can open up more options for what you do next.

More on that over the coming weeks.

Knee osteoarthritis shouldn't mean handing over control, shrinking your world, or living cautiously by default. Movement isn't something being taken away from you: it's something that can be adapted.

If you're unsure where to start, a registered osteopath or physiotherapist can help you unpick what settles, what aggravates, and what feels manageable, and then re-thread movement in a way that fits your body and your life.

The full long thread on this, with all the evidence behind it, is on the website.

People often wait until something's gone badly wrong before they think about seeing an osteopath. A back that's finally given out. A knee that's stopped negotiating. I understand why, we're all busy, and booking an appointment can feel like one more thing on the list.

But bodies rarely fail out of nowhere. There's usually a long, quiet build-up before the loud moment. And that build-up is exactly where I can do the most good, not just patching things up afterwards, but helping you understand what's actually going on before it becomes a bigger problem.

It's not just about who you are today, it's about who you're going to be in 10 years. That's what prevention actually means to me.

You don't need to be in crisis to book in. If something feels a bit off, a bit stiff, a bit not-quite-right, that's a perfectly good reason to come and see me.

Booking online takes a couple of minutes.

This is one of the hardest worries to navigate, and I hear it often.

Exercise remains effective even in more advanced osteoarthritis, and even when long-standing pain complicates the picture. If walking feels like too much, cycling and swimming often remain accessible.

This isn't about forcing movement through pain. It's about choosing movement that respects where you are, without assuming you're fragile.

Movement does not accelerate joint damage. It does not worsen disease progression. It does not wear the knee out faster.

In fact, people who maintain regular physical activity over time are less likely to progress needing a total knee replacement than those who become inactive.

Your knee is not a ticking time bomb. Staying active is one of the most powerful ways to support your future mobility and independence.

High-quality evidence shows that aerobic exercise, walking, cycling or swimming, is the most effective way to improve pain, physical function and quality of life in knee osteoarthritis.

None of these is better than the others. Walking isn't superior to cycling. Swimming isn't inherently more protective. What matters most is choosing something that fits your life, and that you can return to consistently.

The goal isn't a perfect routine. It's one you'll actually keep doing.
