It’s time we stopped calling testosterone a “male hormone”.

Testosterone is a female hormone too. For decades, we’ve simplified human hormones into neat little boxes: oestrogen belongs to women and testosterone belongs to men.

Biology is far more interesting than that.

Women naturally produce testosterone throughout their lives. It is produced primarily by the ovaries and adrenal glands, and it is also formed through the conversion of other hormones within the body.

Testosterone and other androgens are involved in female sexual function and contribute to musculoskeletal health and other physiological processes. Testosterone can also be converted into oestradiol, the main form of oestrogen in premenopausal women, within the body.

Yet many women reach menopause knowing plenty about declining oestrogen and almost nothing about their androgens.

There’s another misconception worth correcting: testosterone does not simply disappear when a woman reaches menopause. Testosterone levels generally decline gradually with age, beginning before menopause, rather than falling abruptly at the final menstrual period. The ovaries and adrenal glands can continue producing androgens after menopause, although ovarian androgen production may decline over time.

So perhaps we need to change the language. Calling testosterone the “male hormone” can unintentionally suggest that it is somehow foreign to the female body. It isn’t.

Just as men naturally produce and need oestrogen, women naturally produce and need testosterone.

That does not mean every menopausal woman needs testosterone therapy.

The strongest evidence for prescribing testosterone to women is currently for appropriately assessed hypoactive sexual desire disorder (HSDD) in postmenopausal women. Current guidance does not support using testosterone routinely for general wellbeing, cognitive symptoms, mood, energy, or musculoskeletal health. We should be careful not to turn an important hormone into another menopause cure-all.

But we should absolutely teach women that testosterone belongs in the conversation.

Oestrogen isn’t exclusively female. Testosterone isn’t exclusively male. They are human hormones.

Perhaps it’s time our menopause education reflected that.
