A decision can be sound and still land badly. When nobody takes the time to explain a record properly, the people it affects are left guessing at the very thing that shapes their future. Reading the full record, naming the hard truths plainly and writing a report a stranger can follow is part of the decision - not admin that follows it.

Should families see the full record, or a summary?

There's a real argument for both. The full record is transparency in its purest form. A summary is honesty someone can actually hold in their hands.

No right answer is hiding here - this one genuinely depends on trust, context and who's asking.

Marking your own homework is not assurance.

When the same team runs the process and signs it off, the tick only proves the answer matched itself. The record was never tested by anyone independent.

That is the gap an inspection finds, and the gap families notice.

Independent scrutiny is not an add-on to good practice. It is the part that makes good practice credible: someone with no stake in the answer, reading the whole file, saying plainly what they find.

Look at those three lines and notice what they never tell you: who did it, what standard they used, how often it was checked, and what changed as a result. They could sit in any report, so they prove nothing in yours.

Read your own records and ask the harder question. If a sentence could sit in any report, it is not evidence.
