October is Breast Cancer Awareness Month, and the most useful thing we can say is short.

Know what is normal for you.

Not a monthly exam with a checklist. Just getting familiar with how your breasts usually look and feel, so that you notice if something changes. The changes worth showing a GP:

A lump or thickening, in the breast or in the armpit
A change in size, shape or outline
Skin that puckers, dimples, or looks red
A change to the ni**le, including it turning inwards, a rash, or discharge
Pain in the breast or armpit that is there all the time

Most changes turn out not to be cancer. Every change is still worth getting looked at, and the sooner the better. Men can get breast cancer too. It is rare, but the same applies.

If you are 50 to 71, you will be invited for NHS breast screening every three years. Go when you are invited, and if something changes in between, do not wait for the next one.

Two questions about ADHD that people are often afraid to ask, answered by Omar, one of our pharmacists.

Can someone have ADHD if they did well at school?

Yes. Academic achievement does not rule out ADHD. Some people develop strategies that carry them through school, and the difficulties only become clear later, when the demands on organisation, time management and executive function increase.

Does having ADHD mean you have to take medication?

No. Medication is an important, evidence-based treatment for ADHD, but it is not the only part of care. Depending on the person, support can also include learning about ADHD, psychological or behavioural approaches, changes to their environment, exercise, sleep and other lifestyle changes. Treatment should be built around the individual, not one size fits all.

At Well Life Clinic, ADHD assessments are either carried out in person at one of our clinics - St Albans, Oxted and Cobham - or virtually from the comfort of your home. We offer private assessments for both adults and children (aged 5-17) or under the NHS Right to Choose (Adults aged 18+ only).

Got a question you have never felt able to ask? Send it to us and we will put it to the team.

Yesterday was World Menopause Day.

Last month we talked about perimenopause, the years of change that can start in your late thirties. This is the other half of the conversation.

Menopause is not a single day. For many women the symptoms carry on for years after periods stop: broken sleep, hot flushes and night sweats, low mood or anxiety, brain fog, joint aches, vaginal dryness, and changes that affect s*x, work and confidence.

None of that is something you simply have to get through. There are options, and the right one depends on you, your history and what matters most to you. That is exactly the conversation a twenty minute GP appointment is for.

It is also a good moment to think about the longer term. Bone health and heart health both change after menopause, and a check-up now is time well spent.

Half term is nearly here. Most state schools in Surrey have a two week break this year, from 19 to 30 October, and in Hertfordshire it is 26 to 30 October.

If you are going away, two key things are worth sorting this week rather than the night before.

Repeat prescriptions. Make sure you have enough to cover the trip, plus a few days spare in case of delays.

Medication in your hand luggage. Keep it in the original packaging, and if you are carrying anything unusual, ask us whether a letter would help.

And if someone comes down with something while the children are off, we are open as usual through half term, with same day appointments and Saturday mornings.

October is ADHD Awareness Month, so we asked Omar Ahmad, one of our pharmacists, what he wishes more people understood.

His answer: ADHD is not simply a problem with attention.

Many people with ADHD can focus extremely well when something is interesting or engaging to them. The difficulty is often with regulating attention, motivation and executive function, rather than being unable to concentrate.

Two myths worth retiring:

"People with ADHD just need to try harder." ADHD is a neurodevelopmental condition affecting executive function, attention regulation, impulsivity and motivation. Telling someone to try harder does not address the underlying difficulty.

"ADHD means you cannot concentrate." Many people with ADHD have periods of very intense concentration, often called hyperfocus. The difficulty is regulating where and when attention goes.

It is also more complicated than "dopamine seeking". Seeking stimulation is not always about pleasure. It can be driven by boredom, frustration, conflict, urgency or fear.

And the strengths rarely get the airtime: creativity, curiosity, spontaneity, lateral thinking, problem solving and enthusiasm. Not in everyone, but too often overlooked.

Why it matters: untreated or under-treated ADHD can reach well beyond school or work, into relationships, employment, finances, mental wellbeing and overall quality of life.

Seen a lot of the sun this summer? This is the month to check your skin.

Most moles are completely harmless. It is the ones that change that are worth a look. After a summer of sun exposure, that's when changes tend to show up.

The ABCDE guide:

Asymmetry, where one half does not match the other
Border that is ragged, blurred or irregular
Colour that is uneven, or more than one shade
Diameter larger than about 6mm, roughly the end of a pencil
Evolving, so anything that has changed in size, shape, colour or feel

Also worth showing someone: anything itching, bleeding, crusting, or not healing.

A mole check is a short appointment with a GP for peace of mind or a closer look.

Not sleeping. Snapping at people you love. Losing words mid sentence. Being told it is just stress.

Perimenopause can start in your late thirties, and it often looks nothing like the version people describe. The hot flushes get all the attention. The rest of it does not.

Things worth mentioning to a GP:
Sleep that broke at some point and never came back
Mood changes, anxiety, or a shorter fuse than you recognise
Brain fog, or losing words and names
Cycles that have changed in length, timing or heaviness
Joint aches, or a body that feels different for no clear reason

You don't have to put up with any of it, and none of it is too small to mention. What it needs is twenty minutes with a Women's Health GP who takes it seriously and looks at the whole picture. Blood tests included where they are useful.

Fever, cough, rash, tummy bug. Every September, the same question at 7am: school or not?

Here is the honest version, following the NHS guidance most schools work to.

Usually fine for school: a mild cough or cold without a fever, a runny nose, a single bout of being sick with no fever, and rashes your child already has a diagnosis for.

Keep them home: a temperature of 38C or above, vomiting or diarrhoea in the last 48 hours, chickenpox until the spots have crusted over, or a child who is simply too unwell to take part in the day.

Worth a GP appointment: a fever that has not settled after three days, a cough still going at three weeks, breathing that looks like hard work, or a child who has stopped drinking fluids.

Straight to urgent care: a rash that does not fade when you press a glass against it, a stiff neck with a fever, or a child who is difficult to wake.

Trust your instincts. If something feels wrong, get them seen.

Two other bits of September admin while you are here. Get repeat prescriptions sorted before you run out, and if you are travelling at half term, travel vaccines are worth booking now rather than the week before.
