ANNUAL REVIEWS

If you have a long-term condition or take certain medications, we will invite you for an annual review during your birth month.

Over the last few years, we have found that some patients have not received their review invitations, partly due to limitations with the electronic system we were using.

We have now introduced a new system and appreciate your patience while we continue to learn and refine the process.

HOW YOU WILL BE INVITED

Invitations will be sent in the following order:

Text message
Email
Letter (if we are unable to contact you electronically)

Text messages and emails will include a booking link that will allow you to book the appropriate appointment with the clinician required for your review.

Changes to some annual reviews

We have made some changes to how certain reviews are carried out, including reviews for:

Diabetes
Heart disease
Stroke
Other related long-term conditions

You will initially be booked for a 20-minute appointment with a member of our nursing team. During this appointment, they will carry out any necessary tests, which may include:

Blood tests
Blood pressure checks
Other relevant health checks

Where possible, the nurse will complete your review during this appointment. If further follow-up is needed, they may:

Arrange a medication review with our in-house pharmacist
Book a further face-to-face appointment
Arrange a telephone consultation to discuss your results
Why are we making these changes?

We hope these improvements will make booking appointments easier and reduce the need to queue on the telephone.

Flu and COVID-19 vaccinations

Where possible, invitations for flu and COVID-19 vaccinations will also be sent through this system. A consent form will need to be completed before your vaccination appointment.

We hope this will help your experience when booking your appointments and help free up the Telephone lines.

Thank you for your continued support and Co-operation

Jo and the Team

FLU VACCINATIONS

Apologies for the long post but a lot of information to pass on.

We have reached that time of year again where we are offering flu vaccination to our patients.

The criteria is the same as previous years.

For September we are vaccinating

πŸ’‰ 2 & 3 year old children & children up to the age of 17 in an "AT RISK" Group

The children vaccine is a simple nasal spray.

πŸ’‰ Pregnant Ladies (from end September)

From 1st October we are able to vaccinate all patients over the age of 65 (or will be 65 by 31/3/2027).

We can also vaccinate patient aged 18 - 64 in an "AT RISK" group.

Patients in a Care or Nursing Home and Housebound patients.

As we did last year we are able to offer patients over the age of 75 or who are Severely Immunocompromised. These can be given at the same time as the Flu vaccination or separately if preferred.

We have started sending reminders out to patients with booking links to appointments. These will continue for the next couple of weeks. However, you don't have to wait to get the link you can book into the clinics that are already on. I

We are holding a Saturday morning clinic as we normally do on 3rd OCTOBER and appointments are available to book now.

As well as the flu and Covid we are vaccinating for RSV, Pneumonia and Shingles. Invitations are currently going our for these.

If you have any questions please ask the reception staff or nurses if you have an appointment.

Many thanks
