August 14–15, 2026 reports အရ Yangon မှာ acute diarrhoea cases တိုးလာပြီး Yangon General Hospital ကို 4 ရက်အတွင်း လူနာ 182 ယောက် admission ရှိခဲ့ပါတယ်။ Cases တွေက China Town တဝိုက် Latha/Lanmadaw ဘက်မှာ concentration များပြီး contaminated water used in food preparation နဲ့ outside/street food ကို possible sources အဖြစ် စုံစမ်းနေပါတယ်။
Official Safety Guidelines for Residents
လူနာတိုင်းအတွက် Nursing Care Plan ဆိုတာရှိပါတယ်။ NCP ကို Five Steps of Nursing Process (ADPIE) နဲ့သွားပါတယ်။
Assessment (Nursing Assessment)
“How dehydrated is this patient?”
WHO guidelines တွေအရ severe dehydration ကို lethargy/unconsciousness, sunken eyes, unable/drinking poorly, skin pinch returning very slowly စတဲ့ findings တွေကနေ ခွဲခြားနိုင်ပါတယ်။
Deficient Fluid Volume related to excessive gastrointestinal fluid loss secondary to acute diarrhoea and/or vomiting, as evidenced by frequent watery stools, thirst, dry mucous membranes, decreased urine output, tachycardia, hypotension and/or other signs of dehydration.
• Frequent watery stools
• Vomiting
• Excessive thirst
• Dry mouth/mucous membranes
• Sunken eyes
• Tachycardia
• Hypotension
• Decreased urine output
• Weakness / lethargy
• Poor peripheral perfusion
Diarrhea related to gastrointestinal infection associated with suspected contaminated food/water exposure, as evidenced by frequent loose or watery stools, urgency, abdominal cramping and increased bowel activity.
• Amount
• Appearance… Watery/mucus/blood
• Vomiting
• Abdominal cramps
• Fever
• Food/water exposure
WHO recommends continued nutrient-rich feeding during diarrhoeal illness rather than unnecessary prolonged fasting.
Nursing Diagnosis 3 ကတော့ Risk for Electrolyte Imbalance ပါ။
Risk for Electrolyte Imbalance related to excessive loss of gastrointestinal fluids secondary to diarrhoea and vomiting.
• Fluid balance
• Intake & Output
• Urine output
• Mental status
• Muscle weakness
• Ordered serum electrolytes
• Renal function
Risk for Shock
Risk for Shock related to severe fluid and electrolyte loss secondary to profuse acute watery diarrhoea.
⚠️ Poor peripheral perfusion
⚠️ Severe dehydration
Risk for Transmission of Infection ပါ။
Risk for Transmission of Infection related to faecal contamination, unsafe water/food and inadequate hand hygiene.
Nursing care
✅ Hand hygiene
✅ Appropriate PPE
✅ Safe handling of stool & vomitus
✅ Safe disposal of contaminated waste
✅ Proper handling of soiled linen
✅ Environmental cleaning & disinfection
✅ Safe drinking water
✅ Food hygiene
Planning should be patient-centred and measurable.
Overall Goals
The patient will achieve and maintain adequate hydration and electrolyte balance, demonstrate reduced gastrointestinal fluid losses, remain haemodynamically stable, remain free from complications such as hypovolaemic shock and demonstrate behaviours that reduce transmission of infection.
Expected Outcomes
Within the appropriate clinical timeframe, the patient will:
• Maintain/improve BP, pulse and peripheral perfusion.
• Demonstrate improving hydration status.
• Maintain adequate urine output appropriate to clinical condition.
• Demonstrate improved thirst and mucous membrane hydration.
• Tolerate ORS/oral fluids where appropriate.
• Show decreasing stool/vomiting losses.
• Maintain or improve electrolyte and renal-function results when monitored.
• Remain free from hypovolaemic shock.
• Patient/family will demonstrate correct hand hygiene, safe-water practices and understanding of warning signs requiring medical attention.
I would organize the interventions into
ASSESS → REHYDRATE → MONITOR → PREVENT → EDUCATE.
1️⃣ ASSESS
2️⃣ REHYDRATE
3️⃣ MONITOR
Vital signs, consciousness, stool/vomiting losses, intake/output, urine output နဲ့ hydration status ကို ဆက်လက်စောင့်ကြည့်ပါ။
4️⃣ PREVENT
Hand hygiene, safe water, food hygiene, appropriate PPE, environmental cleaning နဲ့ safe waste disposal ကို လိုက်နာပါ။
5️⃣ EDUCATE
1. Assess and reassess dehydration
• Monitor BP, pulse, respiratory rate, temperature and mental status according to severity.
• Assess thirst, dry mucous membranes, sunken eyes and peripheral perfusion.
• Monitor for deterioration or signs of shock.
• Assess stool frequency, amount and appearance.
• Monitor vomiting and ability to tolerate oral fluids.
2. Restore fluid and electrolyte losses
For patients who can drink and do not require immediate IV resuscitation:
• Administer ORS according to the prescribed/local protocol.
• Encourage frequent appropriate oral fluid intake.
• If vomiting occurs, ORS may be restarted/given more slowly in small frequent amounts as clinically appropriate.
For severe dehydration or shock:
• Escalate urgently to the medical/resuscitation team.
• Establish/assist with IV access.
• Administer prescribed IV fluids according to protocol.
• WHO guidance for severe dehydration in suspected cholera/acute watery diarrhoea generally prefers Ringer's lactate, when appropriate and available.
• Reassess the response to fluid therapy frequently.
WHO identifies ORS as a key treatment for dehydration caused by diarrhoea.
3. Maintain strict fluid balance
Document:
Intake
• Oral fluids
• ORS
• IV fluids
Output
• Urine
• Stool frequency/estimated losses
• Vomiting
Monitor urine output closely because decreasing urine output can indicate worsening dehydration and impaired renal perfusion.
4. Monitor electrolyte and renal status
As ordered/clinically indicated:
• Na⁺
• K⁺
• Cl⁻
• HCO₃⁻
• Urea/BUN
• Creatinine
Observe clinically for weakness, altered mental status, cardiac rhythm changes and other evidence of electrolyte disturbance.
Red Flags to Monitor 🚨
Escalate immediately if the patient exhibits any of the following:
• Signs of shock (e.g., severe hypotension, tachycardia, confusion).
• Bloody or mucoid stools (dysentery).
• High fever or worsening severe abdominal pain.
• Anuria (absence of urine output) over several hours.
5. Provide appropriate gastrointestinal and nutritional care
• Monitor stool characteristics, including blood or mucus.
• Collect stool specimens when ordered.
• Provide perineal care after diarrhoeal episodes.
• Protect skin from moisture-associated skin damage.
• Encourage/continue appropriate nutrition as tolerated rather than unnecessary prolonged fasting.
• Administer prescribed medications and monitor therapeutic/adverse effects.
6. Prevent infection transmission
• Perform strict hand hygiene.
• Use appropriate PPE according to exposure risk and facility precautions.
• Safely handle stool and vomitus.
• Properly manage contaminated linen and waste.
• Clean and disinfect contaminated surfaces.
• Maintain safe toilet/commode practices.
• Follow contact/enteric precautions according to institutional/outbreak policy.
Safe water, sanitation and hygiene are central measures for preventing diarrhoeal disease.
7. Patient and family education
• Hand Hygiene:
• Food Safety:
Antibiotics
Nurse ရဲ့ အဓိကတာဝန်ကတော့ early recognition of dehydration, appropriate rehydration, close monitoring, infection prevention and timely escalation ဖြစ်ပါတယ်။
Evaluation answers one important question:
“Did our nursing interventions achieve the expected outcomes?”
After interventions, reassess and compare the patient's condition with the baseline assessment.
Desired Evaluation Findings
Hydration improved
• Thirst decreased.
• Oral mucosa becoming moist.
• Eyes/appearance improving where dehydration signs were present.
• Patient tolerating oral fluids/ORS.
Circulation stable
• BP stable/improving.
• Pulse improving toward expected range.
• Peripheral perfusion adequate.
• No evidence of hypovolaemic shock.
Renal perfusion adequate
• Urine output improving/adequate for the patient's clinical context.
• Renal function stable/improving when laboratory monitoring is indicated.
Gastrointestinal losses improving
• Frequency/volume of diarrhoea decreasing.
• Vomiting decreasing or resolved.
• Oral intake improving.
Electrolytes stable
• No clinical evidence of significant electrolyte disturbance.
• Laboratory values stable/improving when measured.
Infection prevention understood
• Patient/family demonstrate appropriate hand hygiene.
• Patient/family verbalize safe-water and food practices.
• Patient/family can identify important red flags.
If outcomes are NOT achieved
This is an important part of ADPIE.
Do not simply continue the same care plan.
REASSESS → IDENTIFY THE PROBLEM → ESCALATE → REVISE THE CARE PLAN
ADPIE is actually a continuous cycle, not a one-time checklist:
🔄 ASSESS → DIAGNOSE → PLAN → IMPLEMENT → EVALUATE → REASSESS
ဒါလေးကတော့ Short to the point ပါပဲ။
A — ASSESS: How dehydrated is the patient?
D — DIAGNOSE: What nursing problems/risks are present?
P — PLAN: What measurable outcomes do we want to achieve?
I — IMPLEMENT: Rehydrate, monitor, prevent complications and educate.
E — EVALUATE: Is hydration improving? Is the patient stable? If not—reassess, escalate and revise the plan.
