LIVEWELL COMMUNITY MOBILE CLINIC (LCMC)

HEALTH AMBASSADOR PROGRAMME

MISSION:
- To raise community-based health ambassadors across Ibadan who will promote preventive healthcare, mobilize residents for the Community Mobile Clinic, identify vulnerable persons early, and facilitate referrals to LIVEWELL MULTI-SPECIALISTS HOSPITAL before medical emergencies or sunden deaths occur.

CORE OBJECTIVES:
1. Prevent avoidable emergencies and sudden deaths.
2. Promote early detection of chronic diseases.
3. Increase health awareness in communities.
4. Mobilize residents for mobile clinic programmes.
5. Build a community referral network for LIVEWELL MULTI-SPECIALISTS HOSPITAL.
6. Establish a health representative in every major community across Ibadan.

ROLES OF LIVEWELL HEALTH AMBASSADORS:

1. COMMUNITY MOBILIZATION:
- Mobilize residents for mobile clinic activities.
- Create awareness before every clinic.
- Distribute flyers and publicity materials.
- Announce programmes in community meetings.

2. HEALTH EDUCATION, Educate residents on:
- Hypertension
- Diabetes
- Stroke prevention
- Kidney disease prevention
- Obesity
- Healthy lifestyle practices
- Regular medical checkups.

3. IDENTIFICATION OF HIGH-RISK INDIVIDUALS:
- Identify elderly persons who is at risk.
- Identify persons with chronic illnesses.
- Identify persons with recurrent medical complaints.
- Encourage them to attend the mobile clinic.

4. REFERRAL FACILITATION:
- Refer community members to LIVEWELL MULTI-SPECIALISTS HOSPITAL.
- Follow up referred patients.
- Ensure continuity of care.

5. COMMUNITY HEALTH SURVEILLANCE:
- Report emerging health concerns.
- Report disease outbreaks.
- Identify vulnerable populations needing intervention.

6. EVENT COORDINATION:
- Assist in venue preparation.
- Coordinate registration.
- Manage crowd control.
- Assist the medical team during outreach.

7. PARTNERSHIP DEVELOPMENT: Liaise with:•
- Community leaders
- Religious leaders
- Market leaders
- School authorities
- Youth groups
- Trade associations etc

8. DATA COLLECTION:
- Maintain attendance records.
- Collect contact details of participants.
- Keep records of referrals.

9. ADVOCACY:
- Promote preventive healthcare.
- Promote healthy living.
- Promote community ownership of the mobile clinic.

10. BRAND REPRESENTATION:
- Serve as the face of LIVEWELL COMMUNITY MOBILE CLINIC within their community.
- Protect and promote the image of LIVEWELL MULTI-SPECIALISTS HOSPITAL.
