Why is doctor follow-up necessary?
We have a gazillion reasons for our poor healthcare outcomes. Many of these barriers are part of our general knowledge, even though we appear helpless before these challenges, but then there are a host of hurdles that we have not recognized at all. One of them is a regular health checkup, also called "follow-up" in medical terminology.
What is a follow-up?
Follow-up is the act of contact between a physician and his patient to evaluate his progress since his last visit. Patients with chronic conditions, like high blood pressure, diabetes, kidney disease, etc., need regular follow with their doctors, usually four times a year, sometimes more. This visit can include a number of things, varying from a simple physical exam to a range of blood tests and imaging studies. The purpose of the meeting is to evaluate the patient's condition, address his concerns, and make the necessary changes to the prescription.
Our patients show a severe lack of interest in follow-up. We believe that, after financial issues, the primary causative factor is lack of education, not in the sense that patients are illiterate, but that our healthcare system fails to emphasize the importance of follow-ups. Awais Zaka

Why do doctors order so many diagnostic tests?
Our healthcare system never fully adopted modern science. Somehow, we got stalled at a specific phase of the medical evolution, this stagnation causing over time more and more confusion. Consequently, our healthcare system split into many sects—science-based modern medicine, herbalists, homeopathic, and numerous others.
Even though the biggest among these factions still remains science-based medicine, the practitioners of this discipline find their hands tied when it comes to the use of biomedical tests and imaging. Most of the time, patients hesitate to proceed with such investigations; occasionally, physicians show reluctance to use the tests because of cultural and economic constraints.
What our patients don’t understand, and physicians often fail to convey, is that just as technology has made our life more efficient and our measures more precise, science has made our diagnostic and treatment abilities more accurate. Gone are the days when physicians used to diagnose the illness with a wise finger and penetrating stare. Instead, to reach the correct diagnosis, we need, in addition to a thorough examination, a meticulous workup, which includes imaging and blood tests.

Relation between socio economic status and health
Our health parameters and illness scores are off the charts. Such horrible health outcomes are not entirely unrelated to our country’s socioeconomic problems. As our government and policymakers are not interested in people’s health, we are left to ourselves to improve our situation.
In a developing country like ours, where institutions are a slave to personalities, where persons in the leadership role use public tax to advance their own interests, and where the economy is rarely used for the common good, it is hard to see any growth in the society or improvement in people’s lives.
The health of any country’s people is strongly tied to that country’s culture, politics, and socioeconomic landscape. As a result, all strata of society are affected—poor to middle class to wealthy.
The poor take the worst hit, though. He is excluded and marginalized, becoming almost invisible—without work, without possibilities, without even means of escape. Our government hospitals are packed with these invisible patients who have no voice, say, or control over their illness. It is hardly possible to blame them for their sickness or ask them to behave better. They are at the mercy of Heaven.
However, the society above the poverty line can significantly contribute toward not only their own health but also to improve the health of the community as a whole.

We often fail to convey to our patients the importance of the medication. Consequently, they take the benefits of medicines for granted and don't hesitate to manipulate the prescription at their whim.
For patients with chronic illnesses—illnesses that never get cured, like high blood pressure and diabetes—such deviation from a doctor's prescription or absent days from medication can result in fatal outcomes.
It cannot be emphasized more that such patients should maintain a regular supply of their life-saving medicines, enough to help them through any political, social, and financial upheavals, events that are very common in our country.
