At Cedar Crest our Nurse Navigator helps patients understand their heart failure care and prepares them to take an active role in managing it at home.

That preparation includes education throughout their stay, scheduling follow-up appointments before discharge, and providing a scale and weight log to support continued monitoring at home. 🏡

Throughout October, Cedar Crest Post Acute is recognizing Breast Cancer Awareness Month. Using pink ribbons and acrylic paint markers, we created images and messages of hope and support for those whose lives have been touched by breast cancer. 💗
