Qualifying Chronic Conditions 2020 | CCM Program Requirements

Patients who are looking to enroll in the Chronic Care Management Program offered by Medicaid must have a minimum of two different chronic conditions to qualify. Below is a list of some of the major qualifying chronic conditions, though there are more conditions that are not listed below. If you are unsure whether your conditions would qualify you for Chronic Care Management program, feel free to contact us at 805-823-0981

For your condition to qualify as a chronic conditions covered by the CCM program, it must:

- Be a condition that is expected to last at least 12 months, or until the death of the patient

- Be a condition that puts the patient at a significant risk of death, functional decline, or exacerbation/decompensation

She had stomach surgery when this COVID 19 crisis began and was frightened. She was hesitant to have this surgery but decided to continue with the procedure. I was able to comfort her and schedule a telemedicine visit with her PCP. Prior to my call she did not know that a non-face to face meeting was possible. On my calls I find out that many patients do not have family or friend support for shopping or medication pickups. Some are eating fast food daily and I help arrange hot meal doorstep delivery as well as set up Medication home drop offs. The CCM work I do make an impact during this pandemic because it allows our staff to continue provide care to the most at-risk patients during a critical time. CCM is important especially now, because the nature of a non-face to face service ensures patients can safely receive healthcare in the comfort of their own home. Patients are very appreciative for the COVID-19 information we are providing. I encouraged all elderly patients to stay safe at home. I made sure they have help to get food or medications and if not, I help to set up deliveries. I educated patients on the proper way to washer their hands and the importance of good hand washing. As well as reinforcing the CDC COVID-19 Guidelines. People need to hear information from healthcare professionals, so they take the information seriously. Many patients appreciate the program as it is not liked a timed doctors visit. We spend quality time answering their questions and helping them stay on top of their chronic conditions.
