Did You Notice? 💙

Sometimes the first signs that an older adult needs more support are easy to dismiss.

You may notice:

• Bills or mail beginning to pile up
• Groceries not being replaced
• More missed or canceled appointments
• The same questions being asked repeatedly
• Changes in grooming or clothing
• Less interest in favorite activities
• Increased isolation
• More difficulty managing medications or schedules

One change by itself may not mean there is a serious problem.

But several small changes together can be a reason to check in, ask questions, and learn more about what may be happening.

Pay attention to patterns—not just one moment.

Early conversations can make it easier to understand what kind of support may be helpful.

One Question to Ask This Week 💙

“If something changed tomorrow, who would you want me to call first?”

It sounds simple, but many families have never talked about it.

Knowing the answer can help clarify:

• Who should be contacted in an emergency
• Which family member should receive updates
• Who the older adult trusts to help with decisions
• Whether important phone numbers are current
• Who should be involved if care needs suddenly change

You do not need to have a long planning meeting.

Sometimes one thoughtful question can open the door to a much bigger conversation.

Start small. Listen carefully. Write the answer down.

What Senior Advocacy Is — and Isn’t 💙

Senior advocacy can be confusing because it means different things to different families.

At Elder Guardian 360™, advocacy may include:

• Helping families prepare questions for appointments
• Organizing care information and follow-up needs
• Helping communicate concerns clearly
• Supporting family decision-making
• Coordinating resources and next steps
• Helping families prepare for transitions or changing needs

Senior advocacy is not:

• Medical diagnosis or treatment
• Legal representation
• Guardianship
• Making decisions for the older adult
• Replacing the family’s role

The goal is to help older adults and families feel more informed, organized, and prepared.

Advocacy is about helping people understand their options and navigate care with greater confidence.

Don’t Wait for a Crisis 💙

The best time to prepare is before something happens.

Families often wait until a fall, hospital stay, sudden health change, or emergency before organizing important information and support.

Planning ahead can give your family time to:

• Organize medications and healthcare information
• Gather important documents
• Discuss wishes and preferences
• Identify emergency contacts
• Review transportation needs
• Know who will help with appointments and follow-up
• Understand what support may be available if needs change

You do not need to have every answer today.

Start with one conversation, one document, or one small step.

Being prepared does not mean expecting a crisis. It means knowing what to do if one happens.

Before Your Loved One Leaves Rehab 💙

A discharge date can come quickly. Before heading home, make sure your family understands what happens next.

Consider asking:

• What medications should be taken at home?
• Were any medications stopped or changed?
• Are there activity or lifting restrictions?
• What equipment will be needed at home?
• Is home health or therapy being arranged?
• When should follow-up appointments happen?
• What warning signs should we watch for?
• Who should we call if there is a problem after discharge?
• Are there new fall or mobility concerns?
• What instructions should we keep for reference?

Write down the answers and ask for clarification if anything is unclear.

A successful transition is not just about getting home—it is about knowing what to do once you get there.

Before a Crisis 💙

Would your family know where to find the important information if something happened today?

In an emergency, families may suddenly need access to:

• Medication lists
• Healthcare provider information
• Insurance cards
• Emergency contacts
• Advance directives
• Power of attorney documents
• Hospital or rehabilitation preferences
• Pharmacy information
• Allergies and medical history
• Important phone numbers

Keeping these details organized in one trusted place can save time and reduce confusion when decisions need to be made quickly.

You do not have to organize everything in one day. Start with the basics and add information as you go.

Preparation today can make a difficult moment easier tomorrow.

Family Conversation Starter 💙

Have you asked your aging parent what matters most to them if their needs change?

Families often talk about appointments, medications, and daily tasks—but not always about preferences.

Consider asking:

• What helps you feel most independent?
• If you needed more help, what kind would feel acceptable?
• Would you prefer support at home or another setting if possible?
• Who do you want involved in important decisions?
• What routines or activities are most important to you?
• Are there things you definitely do or do not want?
• Where are your important documents kept?

These conversations do not have to happen all at once.

Starting early can help families better understand wishes before a stressful situation or crisis forces quick decisions.

Planning is not only about care needs. It is also about protecting preferences, routines, and dignity.
