Today, October 18, 2026, marks two special milestones: my birthday and the official launch of **ER Texas Connect!**

My name is **Stephanie Carter**, Founder of ER Texas Connect.

After more than 20 years in healthcare, I've witnessed firsthand the challenges patients and families face after hospital discharge. From missed follow-up appointments and transportation barriers to medication concerns and limited resources, I realized that **healthcare doesn't end when a patient leaves the hospital.**
