Myth: Formula fed babies sleep longer at night than breastfed babies.

Let's talk about a popular myth I hear parents are being told all the time: Formula fed babies sleep longer at night than breastfed babies.

Is this true? Let's look at the research:

Let's start by realizing that it is perfectly normal for babies to wake frequently at night, and in fact is essential in younger babies for caloric intake and for protection against SIDS.

In fact, research shows us that 79% of 6-12 months olds still woke up up at night, with 61% still needed at least one feeding at night.

Research also tells us that babies who consumed more food during the day continued to wake up as frequently as those who do not - which tells us that "filling baby up" during the day in an attempt to get them to sleep longer doesn't really work.

This is because babies don't just wake up at night just because they are hungry, they wake up for various reasons.

When looking at the research, studies show parents who breastfeed do get more sleep than formula feeding parents, with an average of 30 minutes more sleep.

So while some parents read online, or are told by well meaning friends and family, that feeding their baby formula will help their baby sleep longer, this is not necessarily the case.

I thought this might help some tired parents out there feel supported ❤💤💤💤❤

Why did you decide to NOT sleep train your child? So many of us are pressured to get our baby or toddler to sleep through the night, however there are millions of us around the world who choose to do alternatives to sleep training such as bedsharing or co-sleeping and focusing on how we can rest a bit more during the day than sleep train at night. Why did you decide to do alternatives to sleep training?...

A Shout Out for the Mothers

Hello my dear families. I want to let you all know that in recent weeks of self-isolation I have been resting, working, grieving, and retooling. Many things are uncertain, but one thing is sure: babies keep coming even during times of global crisis! In times of transition and change, families need support more than ever. I am here, I am with you, and you do not have to do this alone! Please contact me at [email protected] to discuss the many ways you can be strengthened and equipped for a positive birth.

'Today I got feeding support from the most unlikely of places, the most surreal moment of my life that had me in tears.

I visited Schoenbrunn zoo in Vienna whilst on a mini-break with my partner and son, we nipped in to see the orangutans at the end of our day who were happily playing in the enclosure so I went to the window for a closer look and sat down by the window so my lb could see the orangutan who was roughly 5/6ft away. She then got up, carried a piece of cloth to the window and sat down with me. She looked directly into my eyes then placed her hand up as if to touch my son I was in awe of this beautiful creature already.

My son wanted feeding and as it was quiet I fed him whilst I sat there, the way the orangutan reacted took my breath away she kept looking at me, then my son then back again. She sat with me for approximately half an hour, kept stroking the glass and lay down next to me as if to support and protect me.

I had to share this because my mind was blown, we may be a species apart but breastfeeding connected us today in a once in a lifetime moment that will stay with me forever. I'm also incredibly lucky that my partner caught all of this on video 💕'

Another incredibly interesting story:

Did you know that women from La Leche League once taught a mother gorilla to breastfeed? The Mamma gorilla had been born and raised in captivity and didn't know what to do with her firstborn, and her baby sadly died. When she next became pregnant, breastfeeding women volunteered to sit beside the gorilla's enclosure and breastfeed their babies, showing the gorilla what to do. When the gorilla gave birth, a volunteer came over and breastfed her baby, showing Mamma gorilla what to do, step by step. Mamma gorilla watched, and then copied, and went on to successfully feed her baby

The Lactation Pharmacist

First up in our pharmacology crash course is half-life.

Half-life refers to the length of time it takes for half the drug to leave your body. Let’s take sertraline (Zoloft) for example. I’m on 50mg daily, and sertraline’s half-life is 26 hours. So, after one day, my 50mg dose is now 25mg, because I have excreted half the dose. After another day, my 25mg dose has been cut down to 12.5mg. After another day, my 12.5mg is 6.25mg ... then 3.125mg ... then 1.56mg ... and so on. In general, it takes 5 half-lives for a 98% of dose of a drug to leave your system. In my case, it takes five and a half days for a single dose of sertraline to leave my body.

So why does half-life matter? Drugs with shorter half-lives are preferred during lactation, because it takes less time for them to fully leave the body. Less time in the body = less time to transfer to breastmilk = less change of side effects in breastmilk-fed infants/children. Drugs with longer half-lives can be used, but if you have a choice between two drugs with different half-lives, the drug with the shorter half-life *might* be the best choice. You also have to consider half-lives of any active metabolites (breakdown products of drugs). In the case of sertraline, there is a weakly active metabolite whose half-life is 62-104 hours. Does this metabolite matter? Probably not much in the case of sertraline, but it sure does in the case of codeine - remember, the active metabolite is morphine. You have to consider the half-lives of both the parent drug and any metabolites.

You also have to consider the half-life of the drug when used in pediatrics. Depending on the child’s age, his/her liver and kidneys aren’t mature enough to handle drugs as well as adult liver and kidneys. The drug could have a longer half-life in pediatrics than in adults, which could result in accumulation of the drug and increased risk of side effects. You have to consider the drug’s half-life once it reaches the infant/child and whether that matters.

A drug’s half-life is one piece to consider when determining if a drug is appropriate during lactation. Next week we’ll discuss time to peak concentration.

While most mothers initiate breastfeeding, far fewer continue to breastfeed exclusively at three and six months, according to the Centers for Disease Control and Prevention (CDC) 2018 Breastfeeding Report Card (https://www.cdc.gov/breastfeeding/data/reportcard.htm).

Melissa made our first session so comfortable and easy. She really put us as ease about being able to breastfeed, and answered all of our concerns, which was especially helpful since our baby was born 1 month prematurely and had just come out of the NICU. Having had a really emotionally and physically hard week with our baby coming so early, we so appreciated having someone who could come to the comfort of our home, and who is just a text away with any questions we have along the way. Thanks Melissa!
