Clinical Bulletin – Morpheus8 Excessive sweat

Excessive sweating, specifically in the axilla, can be devastating, leading to social embarrassment, as well as occupational, physical and psychological distress1. Morpheus8 is a fractional RF device with programmable pe*******on depth and energy delivery. The applicator is intended for use in dermatological and surgical procedures for electrocoagulation and homeostasis. The coated 24 needles can pe*****te into the various levels of dermis and subcutis, where sweat glands coils are localized2, coagulating the tissue and causing irreversible thermolysis of the glands structure.

An ounce of prevention is worth a pound of cure.

This weekend, please make sure you stay well hydrated with lots of water. Stay out of the direct sun for extended periods, if possible. Always wear your broad band sunblock before going out and regularly re-apply throughout the day. Consider wearing loose fitting, light colored clothes to reflect the sun's rays. Try a cooling cloth or gel-filled bandana that helps to chill the blood vessels behind your neck.

🥵🥵🥵Heat Exhaustion Symptoms:
Heavy sweating, Weakness or tiredness, cool, pale, clammy skin; fast, weak pulse, muscle cramps, dizziness, nausea or vomiting, headache, fainting...

First Aid: Move person inside, remove all unnecessary clothing like a jacket or socks. Get them to drink a sports or rehydration drink, or cool water. Cool their skin – spray or sponge them with cool water and fan them. Cold packs, wrapped in a cloth and put under the armpits or on the neck are good too.

😵😵😵Heat Stroke Symptoms:
Throbbing headache, confusion, nausea, dizziness, body temperature above 103°F, hot, red, dry or damp skin, rapid and strong pulse, fainting, loss of consciousness.

First Aid: Call 911 or get the victim to a hospital immediately. Heat stroke is a severe medical emergency.

Please remember - do NOT leave children, pets, or elderly in the car. At no point this weekend nor in any circumstance is it okay to leave a living being in a vehicle... not for just a minute, not in the shade, not with the windows down, not with the car running... all of these scenarios can have mishaps. This is an interesting article about Patient Portal. It is Sunday and I am doing my portal tasks :-)

The world was returning to normal. But there was that patient portal! Like most physicians, I was getting many more messages than I had before the pandemic and was spending multiple hours a day answering them. I told my husband that I could fill my entire day just by responding to patient messages and never actually examine anyone.

Not every message I received required a great deal of thought. Some, like routine prescription refills, took just a couple of moments to approve. But many requests were higher order. Patients would write asking me to adjust their medication doses, comment on consultants’ opinions, review studies other physicians had ordered, and even perform literature searches on novel therapies. Several times a week I would be hit with a page-long recitation of symptoms ending with the dreaded one-word paragraph, “Thoughts?” I was growing angry and resentful, and that is not my norm.

Finally, some hospital systems have begun charging patients for more detailed portal exchanges. Personally, I believe this runs afoul of physicians’ traditional ethics. The medical profession has never been one that charges for services by the minute, and many of the patients I see regularly face financial hardship. The majority of them would be unable to pay for portal use and denying them equitable access to care is unjust.

The pandemic was awful for everyone, perhaps in ways not yet understood, and the administrative burdens of practicing medicine today are enormous. I recently had to fill out prior authorization paperwork for a generic muscle relaxer for a patient with back pain. I had to do another to get a patient their inexpensive B12 tablets. But perhaps there is a lesson in this story. Existing strategies to reduce portal burden may or may not be helpful to individual physicians, but it is possible that the most effective approach is to simply ask patients for help. To remind them that even in an age of quick communication, brevity is important, immediate responses may not be reasonable, and face-to-face encounters remain valuable. Furthermore, while technologies will continue to infiltrate all aspects of people’s personal and professional lives, it is increasingly important to use them judiciously and to avoid those that cause personal strain and disconnection.

In this narrative medicine essay, an internist suffering from portal communication burnout explored several solutions, none of which sat well with him, so he decided to write a letter to his patients explaining his fatigue and asking them to abide by some usage guidelines.
