You can appeal a denial.
You can't appeal a calendar.
Timely filing is the one deadline in billing with no second chance.
Miss it and a perfectly valid claim becomes a write-off. In most cases there's no appeal and no exception. It's just gone.
The windows aren't the same everywhere:
Medicare → 12 months from date of service
Commercial payers → often 90 to 180 days, sometimes less
Medicaid → varies by state
Corrected claims and appeals → their own separate clocks
The claims that miss it usually aren't forgotten. They're stuck:
A rejection nobody saw.
A secondary claim waiting on a primary EOB.
A credentialing gap holding claims "until it's fixed."
Most AR reports sort claims by age.
Age tells you how old a claim is.
It doesn't tell you how much time it has left.
Sort by days remaining instead. Different list. Different priorities.
