The SHORT Pregnancy & Dentistry Update ✅
Analgesics, Antibiotics, Anesthetics, Antimicrobials, & Local Anesthesia

An expert workgroup, convened by the U.S. Health Resources and Services Administration in collaboration with the American College of Obstetricians and Gynecologists and the American Dental Association, developed a consensus statement regarding oral health care during pregnancy.

Oral health care, including radiographs, pain medication and local anesthesia is safe throughout pregnancy. An excellent table listing various pharmaceutical agents typically prescribed by dentists is available within the consensus statement and indicates which ones may be used and ones that should be avoided.

Most pharmacological agents that dentists commonly prescribe are safe for use in pregnancy, but there are some antibiotics, including tetracycline, which should be avoided, and a few analgesics that need to be used with caution.

Tooth #30 ~ Periapical Pathology/Apical Resorption and Overextension of GP
Patient was referred to me from her pediatric dentist. She had ortho treatment for a few years. Her age was 16. Tooth #30 had a large periapical lesion on the distal root with resorption. The pulp was necrotic. Endodontic treatment was performed in one visit. Conefit was perfect. I obturated with warm vertical condensation and biocermic sealer. The postop radograph showed an overextension in the distal canal. Instead of retreatment or surgery I decided to just monitor.
Patient returned in 6 months asymptomatic with very nice osseous repair even in the presence of the overextended gutta percha. In a few years the macrophages will phagocytize the remaining gutta percha.
I believe the bioceramic sealer helped this lesion heal even with the overextended gutta percha.

Tooth #14 - Silver Point Retx
Patient presented in pain on tooth #14. She had a previous root canal performed almost 20 years ago when she was 12 years old. It still had the original stainless steel crown and the root canal was obturated with silver points. The stainless steel crown started to leak. This created the pathology (which is the main reason why teeth fail with silver points ~ corrosian creates a silver-chloride salt and contributes to pathology).
I retreated this tooth in 2 visits with calcium hydroxide. The silver points are usually pretty easy or remove or very difficult.

SHORT TIPS:
* Never use ultrasonics to try to remove silver points. They are too soft and will break deeper in the canal.
* Use chloroform to soften up the seal and use small handfiles beside them.
* Once loose, then use stigletz (small forceps) to grasp and remove them.

This case had an untreated MB2 as well as you can see in the cone-fit. Case finished well.

Teeth #2 & #3 ~ Double Pathology
Patient came in pain on teeth 2 and 3. A LFOV 3D CBCT was taken Carestream Dental. There was pathology with the 3D bone rendering on the buccal of tooth 3. In addition, there was pathology noted on both teeth in the sagital view of the CBCT. Endodontic treatment was performed on both teeth in a single visit. ChlorXtra Vista Apex Dental Products was used as the main irrigation and EndoUltra Vista Apex Dental Products to agitate the irrigants. Both cases were obturated with NeoSealer Flo biocermic sealer Avalon Biomed and single cone gutta percha. Patient reported to be asymptomatic. A 6 month recall shows nice osseous healing taking place.

Patient returned in 2 weeks with minimal pain and swelling. Endodontic treatment was performed in one visit. Patient was recalled in 6 months showing nice osseous healing.

Tooth #9 ~ Iatrogenic Error Repaired
Patient was referred from his general dentist after a root canal attempt on tooth #9. The 2D PAX shows a separated file in the middle 1/3 mesially displaced. This possibly happened due to canal calcification and forcing files into place.
*Tip 1 ~ Use rotary files very gently like a sharp #2 pencil trying not to break the tip*
These can be very challenging to remove at this angle. I used a 3D LFOV CBCT to get more information. I was able to measure where the file was located and to see the PAP more clearly with the bone loss at the apex. The file was removed using the surgical microscope and a very small ultrasonic tip.
*Tip 2 ~ Be careful when removing these files so it does not fall into the canal!*
The canal was cleaned, shaped, and obturated in one visit using bioceramic sealer and gutta percha.

Regenerative Endodontics - SHORT Dental Tips
Did you know it's the heavy metal content, which is most likely responsible for the dark pigmentation using MTA based or calcium silicate materials. Zirconium oxide is often used instead of bismuth oxide, as a radiopacifying agent to reduce staining.
Here are some calcium silicate products that has little to no staining:

Patient came in pain on tooth #12. Notice how narrow the canal space is in the center. This is usually an indication of multiple canals. Endodontic treatment was performed. The canals were located with a sharp DG16 under the microscope. Small handfiles 6 and 8's were used to open the canals. A heat treated glide path file DC Taper @SSWhite 17 was then used to help get working length. EdgeEndo X7 files were used with crown down and NeoSealer Flo Avalon Biomed was used to obturate the canals with single cone technique and gutta percha.

Tooth #13 ~ Retx or Apical Surgery?
Patient came in with discomfort on tooth #13. The diagnosis was previous treatment with a large periapical lesion. The previous root canal looked very well done. I decided not to do an apical surgery and a retreatment instead. A retreatment was performed in 2 visits using calcium hydroxide. Patient returned in 2 weeks asymptomatic. The case was completed with warm vertical condensation using ZOE based sealer. Is it short? I trusted my apex locator over the radiograph.
*Studies show resorption on these cases due to a long standing periapical lesion and the physiological apex could be 3-8mm away from the radiographic apex.
A 6 month recall showed some osseous repair and healing. A 3 year recall showed complete healing. A 7 year recall showed stable healing.
*So what was causing this issue? It was inadequate cleaning and shaping. After locating all canals, irrigation and appropriate cleaning and shaping is paramount to root canal success vs radiographic beauty.

Tooth #14 - Overfill & Healed ✅
Patient came in pain on tooth #14. It had a previous root canal with periapical pathology and a sinus tract. The sinus tract was traced with gutta percha point. A retreatment was initiated and calcium hydroxide placed. Patient returned asymptomatic after 2 weeks. Canals were obturated with warm vertical condensation using ZOE based sealer and gutta percha. The DB canal was overfilled on accident.
I decided not to panick. No surgery. Just monitor. Patient reported to be a symptomatic after 2 weeks with resolved sinus tract. Patient returned in 6 years. Showing very nice healing! The macrophages will remove the rest of the overfilled gutta percha over a period of time.

SHORT ENDO FACTS ON OVERFILLS:
1. DON’T PANICK…Gutta Percha is inert. Healing will occur if canals are well cleaned and shaped even if some obturation material is extruded into the periradicular tissues.
2. ZOE based sealer extruded into periradicular tissues during root canal filling disappears from recall radiographs within 6 years.
3. There is evidence that given enough time postoperatively, all extruded sealer will be removed & even over extended gutta percha.
4. There is evidence that gutta-percha is more resistant to removal than the zinc oxide and eugenol-based sealers.
5. Studies show despite overfill overall success rate is 90% if property cleaned and shaped.

Patient came in pain on tooth 31. It had a large carious lesion and a large PAP. The dx was a necrotic pulp with acute apical abscess. Endodontic treatment was performed in 2 visits with calcium hydroxide. I brought the patient back for completion. There was bleeding in the mesial root. I used bioceramic sealer technology to repair and seal the perforated area.
Patient returned in 6 months with tooth restored, asymtomatic with nice healing.

Tooth #31 ~ Very Cool Anatomy!
Patient came with pain on tooth #31. The diagnosis was a necrotic pulp with acute apical periodontitis. Endodontic treatment was performed in one visit using heat-treated NiTi files and bioceramic sealer. Patient was asymptomatic the next day. I'm confident we will see remarkable radiographic healing in 6 months.

Bioceramic sealers contain calcium silicate and/or calcium phosphate to enhance physical and biological properties such as
1. alkaline pH ~ to promote healing and osteoconductive
2. chemical stability within the biological environment
3. lack of shrinkage
4. prevent micor-leakage if a restoration is lost or tooth non-retored
5. bacterostatic and bactericidal properties
6. can strengthen the root
7. effective against E. Faecalis
8. non-staining

Patient came in with pain on teeth 30 and 31. Tooth 30 had a previous root canal with periapical pathology and tooth 31 had a deep restoration causing a necrotic pulp with acute apical periodontitis. Tooth 30 was retreated and tooth 31 was treated with conventional endodontics. A 6 month recall shows healing is taking place. A 4 year recall shows very nice textbook healing on both teeth.

Patient returned 6 yrs later with pain in the same area. He thought the pain was from tooth #13. However tooth #13 had textbook healing! The issue is now tooth #14. The diagnosis was a necrotic pulp with acute apical periodontitis. Patient rescheduled for treatment on tooth #14.

Tooth #19 - A Very Rare Case 😳
Pateint came in pain on tooth #19. The diagnosis was a necrotic pulp with acute apical periodontitis. She had a history of orthodontics as a child which probably lead to the resorption on the distal root. Endodontic treatment was performed in 2 visits. This was a very rare case that had 3 mesial canals (MB, MM, ML) with separate portals of exits and 2 distal canals. A total of 5 canals all with separate portals of exit. Vertucci type VIII in the mesial root and type IV in the distal root.

Let me give you some lit review...🔥🔥

*In a clinical study of 145 mandibular first molars, Fabra-Campos found four molars (2.07%) with five canals- three in mesial root and two in distal.
**In the four cases, the MMC did not show an independent apical foramen. So this is an extremely rare case you are seeing!

He is also an associate clinical professor at the Medical College of Georgia School of Dentistry. His practice (Apex Endodontics P.C) is located in Smyrna, Georgia which is in the suburb of Atlanta .

Dr. Short has been a featured speaker to youth groups all over the country. His main focus is to encourage the youth to set goals and give them realistic advise on how to achieve them. Dr. Short's daily inspiration and motivation can be followed on facebook and twitter. He is also the author of the book, "Getting to the Root of Your Problem: 365 Days of Inspirational Thinking"

Dr. Rico Short was born in Columbus Georgia on June 29, 1974. He was reared from a single family household by a mother who worked 33 years on the floor in a textile mill. Growing up as a child, Dr. Short said he did not have a lot of "things" but he had a lot of love, prayer, and support from his family. Dr. Short said his mother had to "drag" him to church because he really did not see the importance as a teenager. However, now as an adult, his Christian foundation has become the bedrock for all of his success.

Dr. Short is married to Angela Short who is a dental hygienist by profession. They have two children: Jayla and Ava. Dr. Short attributes his accomplishments to an understanding and loving wife and kids but most of all to his Lord and Savior Jesus Christ.
