Patient came in pain and felt like it was a sinus issue. It was actually a tooth issue. Endodontic treatment was performed on tooth #15 in one visit using Brasseler USA #ScoutFiles. I really like these files. They are heat treated, very strong, and very flexible.

Check out the palatal root heading into the sinus and the curvatures on the roots. That's probably why he thought it was a sinus issue! A root canal can sometimes fix a sinus issue.

Tooth #9 ~ Trauma With External Resoption Repair with Bioceramic Technology
Patient was referred to my office after a trauma involving teeth #8 and #9. The trauma was 1 week ago. Teeth #8 and #9 did not respond to pulp testing. I recommended patient to return in 1 month to retest the teeth. Patient return and tooth #8 responded to cold but tooth #9 did not. In fact tooth #9 was also sensitive to percussion and there was a lesion developing due to the trauma. The necrotic pulp and trauma caused external resorption in the apical 1/3 of tooth #9. Endodontic treatment was performed in one visit on this tooth using bioceramic sealer by Avalon Biomed and gutta percha. Patient returned in 6 months for an evaluation. There is evidence of healing and osseous repair. Endodontic treatment using bioceramic technology can safely be performed in one visit on these resorption cases, promotes healing, and can reverse the resorption process. #DrRicoShort #endodontics #resorption #endodontist #bioceramicsealer #bioceramics #dentistry #trauma #rootcanals

Teeth #12 and #13 - Retrx & Conventional Trx
Patient reported with pain on the upper left side. She had pressure and temperature sensivity. The dx was acute apical periodontitis on tooth #12 and acute irreversible pulpitis on tooth #13. Tooth #12 had a root canal performed years ago and tooth #13 had deep decay. I retreated tooth #12 through her crown and was able to get to the apex on all 3 roots. Studies show Mx 1st PM can have 3 roots in only 6-9% according to Skidmore and Mariusz. In addition, endodontic treatment was performed on tooth #13.

Tooth #14 - A Special Case!
Patient came in pain on tooth #14. The dx was acute irreversible pulpitis with acute apical periodontitis. Nothing obviously different on the preop xrays. Upon access and removing the pulpal tissue I located multiple canals. This was a 5 canal first molar (MB, MB2, DB, P1 & P2). This was a rare case. *Acosta Vigouroux reported 5 canals in 2.25% of maxillary first molars.

Tooth #5 ~ A Cracked Tooth
Patient came in pain on tooth #5. He had an old amalgam present. Pulp testing revealed a necrotic pulp with acute apical periodontitis. Upon access there was a crack noted under the microscope.
*These will not show up on a 2D radiograph or a 3D CBCT*
Endodontic treatment was performed in one visit using Bioceramic Sealer and gutta percha. There are some studies that show this combination can strengthen the integrity of the roots. This tooth had 3 roots and 3 canals with separate portals of exits. Patient was told to have a full coverage restoration as soon as possible.
A cracked tooth is defined as an incomplete fracture initiated from the crown and extending subgingivally, usually directed mesiodistally. The fracture may extend through either or both of the marginal ridges and through the proximal surfaces. Keep in mind if the fracture is small and invisible at its furthest extent (even after staining), it likely continues deeper into the dentin and long term prognosis becomes unfavorable. **In cases of cracked teeth, the patient should be fully informed that the long term prognosis is questionable**. Patients should be advised, that cracks may continue to progress and separate causing a split tooth leading to extraction.
One 2007 study by Tan, Chen and Wong (IEJ) out of Singapore evaluated root-filled cracked teeth with a diagnosis of irreversible pulpitis and determined a two-year survival rate of 85.5 percent. It’s still worth saving!

Teeth 8 & 9 - Pediatric Dental Trauma
This 6 year old patient had a complicated crown fracture on teeth 8 & 9 and open apices. The teeth were restored with composite crowns and patient was sent to my office. Apexogenesis procedure was performed on both teeth using Bioceramic Putty. A 3 year recall shows very nice closure of the apex with little calcification. Both teeth responded vital to pulp tests.
When dealing with a complicated crown fracture please pay attention to the tooth clinically and radigraphically.
SHORT TIPS:
*If there is a pulp exposure and it’s been over 24hrs, the apex is closed then in most cases perform endodontic treatment.
**If there is a pulp exposure and the apex is open please don't just place a composite over it. The pulp must be "treated" properly now by using Bioceramic Technology to maintain vitality.

There are occasions where a wisdom tooth needs to have a root canal in order to save it...especially when it has an opposing tooth in function. However, the anatomy on these teeth can be very challenging...like this one on tooth #17. Endodontic therapy was performed in one visit.

Tooth #4 - Vertical Root Fracture?
Patient had a previous root canal. He was swollen and in pain. At first glance it appears as if the root canal was done pretty well on the x-ray but bony breakdown was evident. Could the root be fractured? There was no probing or mobility. I decided to retreat the tooth using calcium hydroxide for 2 weeks. It was previously treated with Thermofil and there was a missed DB canal. Patient healed very well and swelling resolved.

Vertucci and associates stated that 75% of maxillary second premolars in their study had one canal at the apex, 24% had two foramina, and 1% had three foramina. Less than 1% of maxillary 2nd pre-molars have 3 canals. This patient was in that less than 1%.

Tooth #18 ~ Warning ⚠️ No Puff Zone!
Patient came in pain was a 16 year old female. Tooth #18 diagnosis was a necrotic pulp with acute apical periodontitis. Endodontic treatment was performed in 2 visits because the canals would not stop draining due to the infection. Instrumentation has to be very precise on a case like this and an accurate apex locator is critical! *The apex is directly on top of the inferior alveolar nerve*
Calcium hydroxide was carefully placed. Patient returned asymptomatic 2 weeks later.
**This is a “No Puff Zone”⚠️
I used warm vertical condensation with ZOE based sealer very carefully instead of Bioceramic Sealer.
There are studies that show Bioceramic Sealer in the inferior alveolar nerve creating paresthesia since it doesn’t resorb like ZOE based sealer. I attached the study below by Alves JOE 2020.

Tooth #14 ~ A One Visit Retreatment
Patient had a root canal performed in India 5 years ago. She recently started having pain. She was referred to my office for a retreatment. The diagnosis was previously treatment with acute apical periodontitis. There was pathology present on all 3 roots. A retreatment was performed in one visit using Bioceramic Technology. A lot of files were used to get to the apex! Patient returned in 6 months showing very nice healing on all 3 roots.
*Don't Be Afraid To Do One Visit Retreatments Using Bioceramic Technology*
Lit Review:
1. Single-visit nonsurgical endodontic retreatment presented fewer incidences of postoperative pain in comparison with 2-visit nonsurgical endodontic retreatment based on assessments ranging from 1 day to 1 month. (Yelda Erdem Hepsenoglu JOE 2018)
2. A clinical study on single-visit root canal retreatments on consecutive 173 patients ~ Mean observation time was 29 months. Follow-up assessment revealed 100 teeth (90.9 %) of single visit retreatment as healed and 10 teeth (9.1 %) non-healed. (Eyuboglu TF, Olcay K, Özcan M. Clin Oral Investig. 2017 Jun;21(5):1761-1768)

TOOTH #30 - Separated File At Apex - Remove? Bypass?
Patient presented with pain on tooth #30. It had a previous root canal with some very seriously curved anatomy and a periapical lesion. I decided to retreat this tooth verus doing surgery. I opened the tooth up and located another distal canal using the microscope. I tried to bypass the file in the distal root at the apex but could not (Remember the file will not cause a failure as long as it’s sterile - Crump and Natkin and my old school Kakahashi Stanley and Fitzgerald studies!). It’s bacteria beyond the file that can cause failure. Calcium Hydroxide was placed in all 4 canals. Patient returned in 2 weeks asymptomatic and case was completed. Patient returned in 6 months and healing is evident in both the mesial and distal roots in the presence of the separated file. I did not remove the file or bypass it.

Patient was referred from his dentist. He was in pain. The diagnosis was a previous root canal with acute apical periodontitis. Endodontic retreatment was performed in 2 visits using calcium hydroxide. Patient returned in 2 weeks asymptomatic and the root canal case was completed.
The canal anatomy was very unusual. The frequency of reports on two palatal roots with two canals on a mx 2nd molor is low. Peikoff et al. reported a 1.4% incidence of four separate roots. You are getting a peak at a rare sighting!

A New Study In JOE on CBCT & Detecting Fractures
A fracture width >120 μm has been reported to be better visualized by CBCT imaging which is approximately the size of a 15 handfile. From this systematic review and meta-analysis, it can be concluded that the available evidence is weak with regard to the diagnostic ability of CBCT imaging to identify VRFs in root-filled teeth compared with direct visualization.

He has attained countless awards and recognition in the area of dentistry and motivational speaking. In addition, Dr. Short serves as an expert Board Certified Root Canal Consultant to the Georgia Board of Dentistry. He is also an associate clinical professor at the Medical College of Georgia School of Dentistry. His practice (Apex Endodontics P.C) is located in Smyrna, Georgia which is in the suburb of Atlanta .

Dr. Short has been a featured speaker to youth groups all over the country. His main focus is to encourage the youth to set goals and give them realistic advise on how to achieve them. Dr. Short's daily inspiration and motivation can be followed on facebook and twitter. He is also the author of the book, "Getting to the Root of Your Problem: 365 Days of Inspirational Thinking"

Dr. Rico Short was born in Columbus Georgia on June 29, 1974. He was reared from a single family household by a mother who worked 33 years on the floor in a textile mill. Growing up as a child, Dr. Short said he did not have a lot of "things" but he had a lot of love, prayer, and support from his family. Dr. Short said his mother had to "drag" him to church because he really did not see the importance as a teenager. However, now as an adult, his Christian foundation has become the bedrock for all of his success.

Dr. Short is married to Angela Short who is a dental hygienist by profession. They have two children: Jayla and Ava. Dr. Short attributes his accomplishments to an understanding and loving wife and kids but most of all to his Lord and Savior Jesus Christ.
