Nicole Hoff is Board Certified Physician Assistant (PA) and a graduate of the Yale University School of Medicine’s PA Program. When deciding what kind of PA, she wanted to be she said: "I really want to do something “family friendly” so my kiddos can stop sharing their mom so much. But I also want to be able to pray for my patients, and WITH my patients. I don’t want insurance to dictate the care

I give, and I want the services my patients receive to be affordable. I want to prescribe lavender oil, exercise, and sunshine—just as much as I do metformin, ibuprofen, and prozac. And I want to be respected by coallegues if I say to a patient: 'your healthcare decisions are yours,' 'God is the great physician,' or 'just put breastmilk on it!' "

Before becoming a PA, Nicole graduated with a Bachelor of Arts in Maternal and Child Health specializing in lactation. For over 15 years, she empowered families as a midwife assistant, doula, childbirth educator, and International Board Certified Lactation Consultant. Later while serving as a hospital chaplain, Nicole discovered a passion for helping others who do not know where to begin — families bringing a first child into the world, suffering a loss, or who have no access to care. Driven by her desire to integrate compassionate mental health care intuitively into pediatrics and primary care, Nicole aims to provide affordable, accessible mental healthcare and primary care. And of course, she can’t wait to get back to doing well woman and lactation care visits as well! Nicole is also excited to complete her Doctorate of Medical Science in December through Shenandoah University. Her doctoral project is to find a way to incorporate Adverse Childhood Experiences (ACEs) screening into primary care as a standard of care. In her free time, she enjoys spending time with her husband and 4 children (ages 20, 17, 12, 10) in the North Houston TX area or in serving her hometown community of the Rio Grande Valley. Food pairing is a way to combine 2 or more foods in a single meal or snack to support a healthy blood sugar response—rather than a quick sugar spike.

If you’ve heard of the glycemic index, the idea is similar but more practical.

Whereas the glycemic index gives an idea of how much a specific food will raise blood glucose levels, some perfectly healthy foods (like grapes and pineapple) have a high glycemic index.

With food pairing, it’s possible to still eat foods with a high glycemic index as long as they are paired with foods that slow their digestion and absorption.

Breakfast may be the most important time to try this. For example, if you're having coffee (which is typically low in carbs) with a low carb toast or oatmeal (which will have some carbs), you could pair it with a source of protein, like a hard-boiled egg or some Greek yogurt. This combination helps mitigate the spike in blood sugar that might otherwise result from consuming the pastry alone. Results? You’ll feel less bleh before lunch & will likely have more energy than if you just did coffee & a pastry.

What is a pairing YOU like to do?

Just remember: when it comes to blood sugar regulation, each person is unique. It’s always best to test and to work with a practitioner on a plan that is specific to you!

& I work to get your first month of continuous glucose monitoring for free from the pharmacy.

You’ll then see your current glucose level on your smartphone app for as long as the sensor lasts. You can set certain alarms and settings for your app to alert you to when your glucose is out of range. These ranges vary from person to person. CGMs are mainly for people with diabetes. But healthcare providers may recommend CGMs for other conditions that impact glucose levels, like: Prediabetes & Obesity; or to see if possible fatigue, dizziness, or headaches are symptoms due to fluctuating glucose levels.

Storytime📖+ a plea🙏. As my husband Matt approaches the age at which his father passed away & gets closer to the age his mother also left us, it's a poignant reminder of the unexpected twists life can take. We never envisioned navigating parenthood without grandparents on his side, & the emotions have been a rollercoaster through every stage of raising our kids.

Matt’s dad would’ve walked me down the aisle, would’ve cut the kids birthday cakes with a sword, would’ve tossed them up in the air too high as I held my breath, would’ve two-stepped with his granddaughters, would’ve had the biggest catch alongside his grandsons, & would’ve kept my mother in law living her best life in her cowboy hat.

…. Lots of “would haves” had he had the best in routine healthcare.

But we can only do better, if & when we know better.

🐻 As a physician assistant, it's particularly frustrating to see men enter my practice with suboptimal health habits due to lack of good healthcare. It's a reminder of my father-in-law, lovingly nicknamed “Bear” for his formidable strength, both physical & mental. His passing at 47 was a stark wake-up call that no one, no matter how seemingly healthy, is immune to health issues at some point in time.

👔 This Father’s Day weekend, I urge everyone here to cherish & care for the fathers in your lives by reminding them that their health matters.

These are your husbands, your uncles, your brothers, your cousins, your pastors, your grandparents, your nephews, as well as your step & bonus fathers. Fathers are the backbone of family legacies, the man behind your last name, the providers of support, the tellers of bedtime stories, & the silent strength in times of need.

Sadly, they are also some of my rarest patients. And this is not because they are all extremely healthy & fit as can be. Whether it's the demands of work & family, the fear of doctor's judgments, or reluctance toward medication, many men find reasons to avoid seeking healthcare.

Until…. they find providers like… ME.

No shame. No judgement. No waiting room.
No immediate prostate checks or pushing of meds. No hundreds of dollars to have nothing come of it.

Just a conversation. 

”Sir, do you have any concerns about your health or mental health? Are you achieving all your goals in marriage, parenting, & work? What are you passionate about & are you doing it? What are your biggest struggles & how can I help?”

That’s where I start & we end with a prayer to encourage these men to seek their faith first when managing their healthcare, finances, & relationships.

Why?

Because if I can have a healthy father in the home, I will have a healthy mother & that means happy, healthy children. It starts at the top. It trickles down. We can’t pretend any longer that pride or fear or time or money that is keeping a man from seeing a practitioner is okay.

It’s not okay. 

To these men I would kindly like to say :

“Don’t let there be any ‘would haves’ that your family will miss out on.

Be there for the birthdays & weddings.

We need you to show up as healthy as you can be to care for your family to the best of your ability.

Maybe it’s just a check up or a checked box, but the mothers & children in your home will be grateful for a report that gives them peace.

Turn your ‘should’ve’ seen a practitioner into a completed task in the coming weeks that avoids a ‘would’ve’ one day years from now.”

🥰 So, let's honor our fathers by encouraging them to prioritize their health & maybe that means sharing this post. Let's remind them that taking care of themselves isn't just about them but about being there for the ones they love.
After all, behind every strong family is a healthy father who has taken care of himself.

This Father's Day, let’s celebrate with love, humor, & a gentle nudge towards better health.

📣 My patient panel is just about FULL! I will be taking my LAST few patient sign-ups for 2024 by the end of the month of March.

This means…. if you have wanted to become a patient… you MUST DO SO by FRIDAY 5pm on 3/29.

I am only taking about 3 more families, 5 more couples & about 7 more individuals— so please don’t hesitate to message me or email me at [email protected] so I can send you instructions on how to sign up. (I do have some reserved spots for Holistic Living members, Homebirth, Birth Center & Homeschool families, so please specify when you message me).

As a direct primary care provider who gives out my personal clinic cell phone number for my patients to be able to access me 24/7, I have to limit my patient load.

It’s not because I don’t want to help all.the.people
…. it’s because I want to!

And I realize I don’t have the time to take more patients on & still give my subscribers my 💯 % attention.

All new patients will be required to be “subscribing members” which means paying a monthly fee for 24/7 telehealth care inbetween our in person visits.

Families with kids must sign up the entire family unit.

Couples without kids must sign up together as a couple.

Why ? Because when my patients have my cell phone number, they tend to text me about everyone in their family and not only themselves. Plus it is just good medicine to take care of a family unit.

As a reminder of my upfront fees:

$150 to sign-up as a couple
$150 to sign-up up to 3 kids
$150 for individuals to sign-up
This includes your first 1 hour visits.

Monthly subscription fees :

$50 1st adult
$45 2nd adult
$65 up to 3 kids in home
$50 for the extra kiddos

(I will give discounts for large families, families in ministry, & I barter !)

One off visits for non-subscribers/non-member who just need me briefly will be:

$75 telehealth for sick visits
$25 sports/work/school physicals
$200 annual exam
$50 paper work (exemptions, accommodation letters, referrals etc). 

Nope I don’t take insurance. But annual labs are only about $45 & I work hard to find the best price for your meds. so ya can’t beat this.

**BIG ASK** Been spending a few hours each week at TOMAGWA Healthcare ministries, where we are starting the women’s health program. From day 1, the OB/GYN patients will receive trauma informed care.

The main goals of trauma-informed care in women's healthcare include:

1. Safety: Creating a safe and supportive environment where women feel comfortable disclosing their trauma history and can receive care without fear of judgment or re-traumatization.

2. Trustworthiness and Transparency: Establishing trust between healthcare providers and patients by being transparent about treatment options, respecting patient autonomy, and maintaining confidentiality.

3. Empowerment: Empowering women to actively participate in their healthcare decisions and providing them with resources and support to regain a sense of control over their lives.

4. Collaboration and Mutuality: Collaborating with patients to develop personalized care plans that address their unique needs and preferences, and fostering a sense of mutual respect and partnership between healthcare providers and patients.

5. Cultural, Historical, and Gender Sensitivity: Recognizing and addressing the impact of cultural, historical, and gender-related factors on women's experiences of trauma and healthcare seeking behaviors.

6. Resilience and Strengths-Based Approach: Recognizing and building upon women's strengths and resilience, and focusing on promoting healing and recovery rather than simply managing symptoms.

If you're facing difficulties affording your medication 💊, please don't hesitate to reach out to me 📩. With the start of the new year, it's common for my patients with insurance to have deductibles reset, and this might impact your medication costs 📊.

Please remember, ☝️ it's crucial not to skip your medications. In most cases, I can assist in finding a coupon, patient assistance, compounding , cash pay options, or alternative solutions (like crowdsourcing) to ensure your health needs are adequately met 🩺💙.

If I need to help you pay for a months supply of a crucial med I will do so & we can barter something/anything (I love edible trades). My patients do not go without! Your well-being is my top priority! 🥰

(NO, I do not take health insurance for services — as I am a cash pay Direct Primary Care provider. But often my patients do have insurance and can use it for medication, imaging, etc.)
