Nicole Hoff is Board Certified Physician Assistant (PA) and a graduate of the Yale University School of Medicine’s PA Program. When deciding what kind of PA, she wanted to be she said: "I really want to do something “family friendly” so my kiddos can stop sharing their mom so much. But I also want to be able to pray for my patients, and WITH my patients. I don’t want insurance to dictate the care

I give, and I want the services my patients receive to be affordable. I want to prescribe lavender oil, exercise, and sunshine—just as much as I do metformin, ibuprofen, and prozac. And I want to be respected by coallegues if I say to a patient: 'your healthcare decisions are yours,' 'God is the great physician,' or 'just put breastmilk on it!' "

Before becoming a PA, Nicole graduated with a Bachelor of Arts in Maternal and Child Health specializing in lactation. For over 15 years, she empowered families as a midwife assistant, doula, childbirth educator, and International Board Certified Lactation Consultant. Later while serving as a hospital chaplain, Nicole discovered a passion for helping others who do not know where to begin — families bringing a first child into the world, suffering a loss, or who have no access to care. Driven by her desire to integrate compassionate mental health care intuitively into pediatrics and primary care, Nicole aims to provide affordable, accessible mental healthcare and primary care. And of course, she can’t wait to get back to doing well woman and lactation care visits as well! Nicole is also excited to complete her Doctorate of Medical Science in December through Shenandoah University. Her doctoral project is to find a way to incorporate Adverse Childhood Experiences (ACEs) screening into primary care as a standard of care. In her free time, she enjoys spending time with her husband and 4 children (ages 20, 17, 12, 10) in the North Houston TX area or in serving her hometown community of the Rio Grande Valley. How Does it Work?
LDN first binds to opioid receptors. In doing so, it helps to displace the body’s naturally-produced OGF. As LDN displaces OGF receptors, affected cells become OGF-deficient and, as a result, three vital processes occur:
1-Receptor production is increased to try to capture more OGF
2-Receptor sensitivity is increased to capture more OGF
3-Production of OGF is increased to compensate for the perceived shortage of OGF
Since LDN will only block OGF receptors for three to five hours, the body experiences a rebound effect—which greatly increases the production and utilization of OGF. Once the LDN has fallen off the OGF receptors and excreted, the increased number of endorphins bind to the now more-sensitive and more-plentiful receptors. As a result, these new and improved receptors assist in regulating cell growth— promoting healing, reducing inflammation, and increasing immunity and autophagy.

​A cohort study showed additional support “Low-dose naltrexone reduced fibromyalgia symptoms in the entire cohort, with a greater than 30% reduction of symptoms over placebo.”

A case report and discussion published in Cureus: “Clinical trials have proposed a unique mechanism(s) allowing LDN to affect tumors including non-small cell lung cancer (NSCLC) at the cellular level by augmenting the immune system.”

A retrospective study published in the Journal of Clinical Psychopharmacology evaluates links between LDN and quality of life in 215 MS patients :

Crohns/Ulcerative Colitis:
A study of Crohn’s Disease patients published in the American Journal of Gastroenterology showed that after 4 weeks on 4.5mg LDN, 81% of patients exhibited a response to therapy and 67% achieved a remission (P < 0.001). Improvement was recorded in both quality of life surveys with LDN compared with baseline. No laboratory abnormalities were noted. The most common side effect was sleep disturbances, occurring in seven patients.

Inflammation: ​
Clinical Rheumatology published a review of multiple studies stating their findings that naltrexone “reduces the production of reactive oxygen species and other potentially neuroexcitatory and neurotoxic chemicals…and reduces ESR (erythrocyte sedimentation rate) a common marker of inflammation.”

Weight loss:
A study on hyperandrogenic women found “that using Naltrexone actually helped to lower fasting insulin levels by up to 40%.”

A study of women with ovarian failure “who were treated with LDN resulted in a 74% pregnancy and live birth rate.”

1. Glial cell modulation (Younger et al, 2014)
2. Tumor cell growth suppression (Donahue et al, 2011; Liu et al, 2016)
3. Increased Interleukin (IL-2), a major anti-inflammatory cytokine (Zijian et al, 2018)
4. Induces autophagy (Pukab and Lansberg, 2018)
5. Reduction of oxidative stress (Almansa et al, 2013)

Exciting news! 🎉 A recent study published in the Journal of the American Academy of Physician Assistants shows that the public has a super favorable opinion of Physician Assistants (PAs) like me! 🙌

🌟 The research, conducted by NCCPA, revealed that nearly 67% of people have been treated by a PA. Guess what? A whopping 91% of them were super satisfied with their experience and 85% would choose to see a PA again for their medical needs.

🌍 Not only do people trust us, but they're also willing to recommend PAs to others—71.8% would do so! 👏 The study also goes beyond just medical settings, giving a comprehensive view of how the general public feels about PAs.

📊 The survey included more than 1,400 participants across the U.S., representing all ages, sexes, races, and more. Even those who haven’t seen a PA yet are mostly open to it; only 4.3% said they wouldn't consider it.

📚 While the public seems to trust PAs, there's still room for education—about 20% of the people said they needed to learn more about what PAs can do.

This is a specialized branch of cognitive behavioral therapy (CBT) designed specifically for those grappling with post-traumatic stress disorder (PTSD). If you've experienced trauma, CPT offers tools to recognize and challenge any negative beliefs or thoughts stemming from that experience. 🧠

Aims of CPT:

🎯Alleviate PTSD Symptoms: PTSD can manifest in various ways - from distressing memories and avoiding reminders of the trauma, to feeling constantly on edge or distant from loved ones. Together we can lessen these symptoms across the board.🌈Overcoming "Stuck Points": Sometimes, after trauma, we might hold onto rigid beliefs that hinder healing. CPT assists in identifying these beliefs and offers strategies to challenge them, making the healing process smoother. 🌈

🙏 This time of year is bittersweet for me. It is the celebration of life of my daughter, who is turning 13 next week. But it is also a time when I mourn the loss of her twin that we lost in pregnancy. October is Pregnancy and Infant Loss Awareness month.

🌈 The term "rainbow baby" holds special meaning for many—it's a child born after a difficult period of infertility, miscarriage, or loss. This serves as a symbol of hope and recovery, much like a rainbow after a storm. 🌦️

It's essential to recognize that many people face this struggle; in fact, studies indicate that 1 in 4 pregnancies ends in miscarriage. 📊

While society often expects that the next step is to try for another child, it's important to highlight that a "rainbow" can mean different things to different people. 🌱

After experiencing loss or infertility, well-meaning friends and family will say, "Don't give up." Although the intent is good, this can make it seem like there's only one way to heal or find happiness. 🗝️

Loss can make you feel like you're not in control of anything. But even when it feels like you've lost all control, you still have the power to decide what healing looks like for you. 🌟

👶Childhood vs Adult ADHD: It's the same condition, but looks different. In kiddos, think hyperactivity and trouble focusing.

For us adults? More like forgetfulness and procrastination. 📆

💡What Causes It?: It's likely a mix of genes and environment. Not likely because you ate too much sugar or watched too much TV as a kid, although diet, exercise, and lifestyle play a big role. 🍭📺

👫Boys & Girls: ADHD is often spotted in boys but can be missed in girls because symptoms appear differently, which means women have often been undiagnosed for many years.

🛌Sleep Hygiene: Adequate sleep is crucial for cognitive functioning. Poor sleep patterns can exacerbate ADHD symptoms, and improved sleep hygiene can have a beneficial effect.
🫁Biofeedback and Relaxation Techniques: Methods like deep breathing, progressive muscle relaxation, and guided imagery can sometimes help in reducing stress, which can exacerbate ADHD symptoms.

🌟RICE Method - The OG
REST*: Time to kick back and relax, folks! Your sprain isn't going to get better with you acting like a Marvel hero. 🦸‍♂️

❄️ ICE: This is your chill pill. Literally! Ice that sprain for 10 to 20 minutes, and then repeat every few hours.
