Telemedicine and the pandemic!

When i first heard about the practice of Telemedicine, I have to admit it was a bit unsettling to me.....

I remembered all that I learnt as a Medical student during the first few weeks of my clinical rotation.

You have to observe the patient as they walk into the room re: their gait, the expression on their faces, their skin etc. Perhaps the patient is limping, maybe he had polio as a child, is she covered all over in a rash?, Is the patient in a wheel chair, how about the flat affect you see on a person's face when they are so depressed that they don't care anymore about anything?.

And of course you have to touch the patient. It is all about "the magic touch". Isn't it? When your P*P, whom you respect and admire, touches you with that Stethoscope, checks out your heart and lungs, and presses all the problem areas, from head to toe, you feel better already. Even as a child, I loved going to my doctor's office, I loved when he examined me, smiled at me and told my father that I was doing great! And asked about all my school activities and achievements. It was such a positive feeling!

Yes, even though I practiced Telemedicine for some years before the Pandemic, I was one of those skeptics who did not think this was medicine at its best, until 2020.

With the influx of the Corona virus, the death tolls in the US and around the world, amongst my patients, friends, families and neighbors, I wondered how all of this will affect the practice of medicine.

My hospitalists friends were scared to go to work. My closest friend who is an ICU attending was so worried about catching the virus himself that even after a hard days work, for the first time in his life, he experienced the frustrations of nightly insomnia and anxiety. My neighbor, an RN would spray her hospitalist husband every evening in the garage with a bleach mix once he came home from work, and have him totally undress in the garage, then take a shower in the visitor's bath downstairs before going upstairs to meet the kids.

As if the virus itself was not scary enough, there were so many people i knew being diagnosed with life changing diseases... colon cancer, lymphoma, breast cancer, multiple myeloma, kidney failure etc. And of course the thousands of phone calls, of people needing all kinds of prescriptions to prevent and treat the virus, from a Z pak to steroids, hydroxychloroquine, ivermectin.... the list is endless.

Call days became a nightmare... from getting 1-2 calls per week, on one particular friday evening, i got over 20 calls from patients... and that was the first time, I really did rethink my profession as i climbed into bed totally exhausted and overwhelmed!

The world was truly hit by COVID. But the story is not over. As all health care workers were forced to rethink how we do business, as vaccinations emerged in 2021, and we all started masking, washing our hands and sanitizing, cut down on non essential outings and turned to the internet and phones to communicate and connect with loved ones, the beauty of Telemedicine emerged.......

It is truly amazing, how the US and other countries have turned to Telemedicine and modern day technology to deliver a unique, patient centered care, available to almost everyone right in the comfort of their home!

I can only marvel at the gains and beauty of virtual health: when I can talk to my patient while they drive to work or late at night while they are already in bed, be able to hear them wheezing, send in medications to the 24 hr pharmacy a few miles from them, read their grateful reviews the next morning and feel so blessed to be able to take care of them, without worrying that i myself will contract the virus.

Yes, I can truly say at this point, that I love being a physician and I am truly grateful for the opportunities that Telemedicine has provided and I look forward to the future of Telemedicine in a modern day era.
