This year’s Harvest of Hope event was an incredible reminder of what can happen when a community comes together with one shared purpose: supporting families and connecting neighbors to the resources that help them thrive.

Thanks to our partners at Healthy Alliance, Watertown Urban Mission, and Samaritan Medical Center, and every volunteer who showed up with compassion and energy, we were able to serve families across Watertown and create a day filled with dignity, warmth, and connection.

We’re honored to support New York’s Health-Related Social Needs (HRSN) program and help ensure nutritious, medically tailored meals reach the people who need them most.

This weekend, our team at ModifyHealth was honored to stand alongside Healthy Alliance, Watertown Urban Mission, and Samaritan Medical Center to support families across Jefferson County.

On Saturday, we helped distribute 1,000 free turkeys to local Medicaid members — a powerful reminder of how much this community shows up for one another during the holidays.

But this event was about more than a meal.

It was a chance to connect people with New York’s Health-Related Social Needs (HRSN) program and the broader Social Care Network — resources that can provide free healthy meals, housing and utility assistance, transportation, and other supports that too often fly under the radar.

At ModifyHealth, our mission has always been rooted in making healthy living easier and more accessible. Moments like this bring that mission to life in the most meaningful way.

A huge thank you to our partners and volunteers who helped make this event possible — and to the community members who trusted us to be part of their holiday season. We’re grateful for the opportunity to serve.

Join us for a drive-through turkey giveaway to support our NY Medicaid community! We’ll be sharing 1,000 free frozen turkeys with Medicaid members — available on a first-come, first-served basis. While you're there, stop by and chat with Healthy Alliance and their network partners to get help with food, housing, and transportation needs.
