The New York Times just ran a piece calling geriatric care managers the best kept secret in elder care. This is the work I do every day.

Most families only find out this kind of help exists once a parent is already in the hospital. By then everyone is making big decisions in a hurry.

Swipe through for what a care manager does, what it typically costs, and what families in the article’s survey said afterward.

One point worth pausing on is the care manager’s independence. Ask any advisor how they get paid, because it shapes the advice you get.

If you are caring for a parent from a distance, or you can feel a crisis coming, I would love to talk it through with you.

Most families don’t ask about Medicaid until the money is almost gone. By then, there’s no time left to plan, only to react.

Before you tour or sign anything, ask:

• Does this community accept Medicaid?
• Is it only in certain units, after a waiting period, or not at all?
• If it isn’t accepted, will your parent have to move?
• How long does the private-pay runway really last, and what happens in the gap before Medicaid approval?

A forced move during a health crisis is one of the hardest transitions a family can face, and one of the most avoidable.

“I don’t want to be a burden.”

If your parent has ever said this to you, you’re not alone, and it’s not a sign to back off. It’s a sign to start the conversation.

Here’s what I tell families:

→ Ask what they’d want their later years to look like
→ Decide who steps in, and who the backup is
→ Put their wishes in writing
→ Talk about money *before* you need care
→ Make sure your family has a plan, too

Planning isn’t about becoming a burden. It’s about making sure the people who love you aren’t left figuring everything out in a crisis.

Start with one question: “What would you want me to know if you ever couldn’t tell me yourself?”

💜 Save this for the conversation you know you need to have.

Planning ahead is the greatest act of love.

“I’m fine” is the most common thing I hear from parents and the least reliable. Mail, fridge, movement. Three things to check on your next visit, and how to bring it up kindly.

I love how they discuss caregiving as a partnership.

As we mark World Alzheimer’s Day, more than 7 million people over age 65 in the US are living with Alzheimer’s disease — a figure that is projected to almost double in the next few decades. And more than 12 million people are caregivers to loved ones with Alzheimer’s or other dementias. There’s no sugarcoating a disease that results in the widespread loss of brain function, and no number that captures the heartbreak of seeing a loved one decline. And yet, there is reason for hope.

That’s because research shows that Alzheimer’s-related brain changes can begin up to 20 years before symptoms set in, highlighting the importance of investing in our brain health and addressing modifiable risk factors throughout adulthood.

This is a 20-year window of opportunity. As with so many chronic conditions, research is showing that with brain health, our DNA is not necessarily our destiny. We have more power than we previously thought to shape our brain health, both day-to-day and in the long run.

Things I wish someone had told me. So I’m telling you.

A big part of why I started LifeCare Concierge is that there were things I wish I had known when we were going through all of this with my dad. This business, and this community, is about sharing all of it so you walk in a bit smarter than I was and armed with the information you actually need.

A few to start:

A power of attorney dies with the person. Once someone passes, that document does nothing. The estate plan takes over, so make sure there is one.

Get on the assisted living waitlist before you need it. Communities you’d actually want have wait times, and if you start in a crisis, you take whatever has a bed.

You can tell the hospital “this discharge is not safe.” Those words trigger a formal review. You have more say than most families realize.

If there’s something you wish you’d known sooner, tell me in the comments and I’ll cover it.

Planning ahead is the greatest act of love.

Did you know Medicare doesn’t pay for long-term care?

It covers rehab after a hospital stay, up to 100 days and only the first 20 in full. It does not cover help with bathing, dressing, meals or supervision, which is what most older adults need for years. That bill falls to savings, long-term care insurance, or Medicaid once the money runs low.

Comment MEDICARE and I’ll send you a breakdown of what’s covered, what isn’t, and who pays.

Did you know your mother can spend three nights in a hospital bed and still count as an outpatient? It’s called observation status. Medicare only covers rehab after three inpatient nights, so under observation those nights don’t count and the rehab bill comes to you. Ask on day one whether she’s been admitted, and ask again if anything changes.

Comment DISCHARGE and I’ll send you my hospital discharge checklist.
