Exercise helps relieve joint pains due to aromatase inhibitors - a narrative review.

Abstract:
Aromatase inhibitors are prescribed in breast cancer due to their associated lower rate of cancer recurrence compared to tamoxifen. However, aromatase inhibitor-induced arthralgia (AIIA) is one of the leading causes of treatment nonadherence, increasing the risk of cancer recurrence. The pathophysiology of AIIA is poorly understood, and although current recommendations for AIIA include lifestyle changes and analgesics depending on the severity of symptoms, there is no established effective treatment. The aim of this study is to explore the presentation and mechanism of AIIA and investigate the feasibility and efficacy of different exercise interventions (aerobic, resistance, aerobic and resistance combined, and yoga or tai chi) in patients with AIIA to guide the development of formal exercise prescription guidelines. Findings indicate that a mixed-modality regimen of aerobic and resistance exercises is feasible and safe and may serve the most benefit in improving joint pain, functionality, and quality of life. More specifically, the weekly regimen should consist of 150 min of aerobic exercise with two sessions of at least six resistance exercises, 8 to 12 repetitions, three sets each. Supplementary yoga and tai chi may be recommended twice a week depending on a patient's target symptoms. Yoga was associated with improved physical functionality, whereas tai chi was related to improvements in mental health. However, the feasibility and impact of combined aerobic and resistance exercise protocols with yoga or tai chi in our target population were not investigated in this review. The use of large, randomized controlled trials is recommended for future studies.

Thank you very much to the Association of Ballet Academies, Philippines, Inc. (ABAP) for having us at their first Dance Convention held at St. Paul College Pasig! We had a session on dance injury and prevention (so much to discuss… we ran overtime) and another on first aid/emergency preparedness for ballet studios. Then we learned all about pointe shoes (as a non-ballerina, I was quite surprised at the variety of brands and models).

This two-day convention has two tracks, one for actual ballet classes/workshops and another for lecture-type learning sessions. Aside from our health-related bits this morning and pointe shoes, other talks were on social media engagement, nutrition, mental health and theatrical makeup.

Congratulations to ABAP for a successful convention!

My biggest takeaways on the “big picture of Western music history” here are: music is shaped by the interplay of cultures (epsecially rich contribution from the marginalized… but we must be aware of cultural appropriation), and “Western music” isn’t strictly “Western”… it is a dominant type of music but not “pure” because of all the outside cultural influences. Excellent read.

3rd PARM Quarterly Scientific Conference

Physiatry has synapsed with Lifestyle Medicine last May 20, 2023 at The Westin Manila, and synchronized with Human Kinetics in Sports and Exercise last September 1 to 3, 2023 at the Marriott Clark.

Let us now champion the rhythm and rhyme of Dance Medicine with SYMPHONY: Physiatry & Dance Medicine. This scientific program will showcase “Staccato: Clinical Case Conference”.

The panel discussion highlights the case of a dancer with musculoskeletal injuries, and the corresponding assessment and management with a Physical & Rehabilitation Medicine (PRM) approach, and a Rheumatology approach. The stellar faculty featured are distinguished physiatrist Dr. Maria Lourdes Corazon Fernandez-Tapang (Vice-President, PARM NCR Chapter), and exemplary rheumatologist Dr. Sandra Tankeh-Torres (President, Dance Medicine Philippines), with a masterful moderator Dr. Paschenelle Celis (President, PARM NCR Chapter).

The video clip is meant to be funny and all (and it is), but “normal person” has a large range as well… imagine a small adult’s hand stretching to get to the same 8-note octave span. Much more stretching involved. This illustrates why being female (i.e., assigned female at birth) is a risk factor for performance-related musculoskeletal disorders in musicians.
