After an unsuccessful IVF cycle, moving quickly toward the next treatment can feel like progress.

But before asking what else can be added, I would first want to understand the previous cycle.

What happened at each stage?

What worked as expected?

What was different from what we expected?

And if the next plan changes, what is the reason for that change?

Sometimes the review does not reveal one single explanation.

That does not make the review useless.

The purpose is to make the next decision more informed — not simply more complicated.

Understand before you decide.

After a failed IVF cycle, it is natural to expect a completely different plan.

But “different” does not always mean “better.”

Before changing everything, ask:
What worked as expected?
What should stay the same?
What may need to change — and why?

Repeating a reasonable part of the plan is not automatically poor care. Adding more medication, tests, or treatment is not automatically progress either.

Every important change should have a clear purpose.

After an unsuccessful IVF cycle, many patients understandably ask:

“What are we going to do differently next time?”

It is an important question — but there is another question worth adding:

Why?

Some parts of the previous treatment may have worked as expected and may not need to change. Other parts may deserve reconsideration.

The aim is not to create the longest or most complicated treatment plan.

It is to understand what will stay the same, what may change and the reasoning behind those decisions.

One failed IVF cycle does not automatically give us a diagnosis.

It does not, by itself, prove poor egg quality, an immune problem, or recurrent implantation failure.

“Failed IVF” tells us what happened.
It does not always tell us why.

The better question is:
“At which stage did the cycle differ from expectations?”

Sometimes we can identify a possible limiting factor. Sometimes there is no single certain answer.

What matters is avoiding unsupported conclusions from one negative result.

The pattern matters more than the number.

After repeated IVF failure, it is natural to focus on how many attempts you have had. But the number alone does not explain what happened.

Did the same difficulty appear each time? Was ovarian response lower than expected? Were few eggs mature? Was fertilisation low? Did embryo development stop at a similar stage or was every cycle different?

A careful review should ask:

“Where did each attempt differ from expectations, and was there a repeated pattern?”

The aim is not to force one explanation. Sometimes a pattern is visible; sometimes there is no single clear cause.

A u përsërit e njëjta vështirësi? A kishte pak vezë të maturuara? A ishte fekondimi i ulët?

After a failed IVF cycle, adding a new test or treatment can feel like progress.

But a longer treatment list is not always a better IVF plan.

Current guidance does not support the routine use of many IVF add-ons after repeated failure.

That does not mean every additional test or treatment is wrong. An option may be reasonable when there is a specific medical reason for using it.

Before accepting an IVF add-on, ask:

1. What problem are we trying to solve?

2. What evidence shows this may help someone with my history?

Every meaningful change in your next IVF plan should have a clear purpose.

What actually counts as one IVF cycle?

This matters because an embryo transfer and a complete IVF cycle are not the same thing.

An egg retrieval cycle includes ovarian stimulation and egg collection.

An embryo transfer cycle is one procedure in which an embryo is transferred into the uterus.

A complete IVF cycle means one stimulation and egg collection, followed by the transfer of all suitable fresh or frozen embryos created from that collection.

So, one egg collection may lead to several embryo transfers.

That is why saying, “I’ve had three IVF cycles” may mean very different treatment histories.

Before asking whether you have had “too many” cycles, write down your:

• stimulations
• egg collections
• blastocysts
• embryo transfers

Count the steps clearly before you judge the result.

Three failed IVF cycles can feel like a final answer. But the number alone does not tell the full story.

First, ask what you are counting:
Were they three embryo transfers from one egg collection or three separate stimulation and egg collection cycles?

These are different treatment histories.

Age at egg collection, egg source, ovarian response, mature egg number, fertilisation, embryo development, transfer history, medical risks and the personal cost of continuing all matter.

The better question is not only:
“How many times have I tried?”
It is:
“After reviewing what happened, is another attempt medically reasonable, personally acceptable and supported by a clear plan?”

Save this question for your next IVF review.

Should you stop IVF after three failed cycles?

There is no universal number that tells everyone when to stop. But that does not mean everyone should continue either.

First, ask: **What am I counting as three cycles?**

Three embryo transfers after one egg collection and three separate stimulations with three egg collections are different treatment histories.

Before deciding what comes next, we need to look at the whole picture: age at egg retrieval, egg source, ovarian response, embryo development, transfer history, medical risks, and also the personal cost of continuing.

So instead of asking only:

“How many times have I tried?”

A more useful question may be:
“After reviewing what happened, is another attempt reasonable—and is there a clear plan for it?”

How many failed IVF cycles are too many?

“This was my third failed IVF cycle. Should I stop?”

After repeated IVF failure, it is natural to feel tired, worried and uncertain about the next step.

The honest answer is:

There is no single number that is “too many” for everyone.

The number of attempts matters, but it does not tell the whole story. First, we need to understand what we are counting.

One IVF attempt may mean an egg collection. For someone else, it may mean one embryo transfer. A complete IVF cycle includes one egg collection and all fresh and frozen embryo transfers using embryos from that collection.[1]

Therefore, three transfers from one egg collection are not the same as three separate egg collections without a blastocyst.

We also need to ask where the process differed from what we expected:

• Were fewer eggs collected than expected?
• How many eggs were mature?
• Was fertilisation lower than expected?
• Did the embryos stop developing?
• Did implantation not occur?
• Was there an early pregnancy loss?

Failed IVF is an outcome. It is not a diagnosis.

Large studies show that some patients may achieve a live birth after more than three or four cycles. However, the chance is different for every patient. It depends on factors such as age at egg collection, whether own or donor eggs are used, the number of mature eggs, embryo development and previous results.[2]

Repeated failure deserves a careful review. But it should not automatically lead to many expensive tests or IVF add-ons. Some add-ons do not have enough evidence to support their routine use.[3]

Before accepting another test or treatment, ask:

“What problem are we trying to solve, and what evidence suggests that this problem exists in my case?”

Continuing, changing the plan, taking a break or stopping IVF may all be reasonable choices.

A further attempt may still be medically possible, while the emotional, physical or financial cost has become too high for you. Both can be true.

Your decision does not measure how much you wanted a child.

In my new YouTube video, I explain how to review previous IVF attempts and what to consider before making the next decision.

Sa tentativa të pasuksesshme IVF janë shumë?

“Kjo ishte tentativa ime e tretë e pasuksesshme IVF. A duhet të ndalem?”

• A ndaluan embrionet së zhvilluari?
• A nuk ndodhi implantimi?
