Rebound pimples when coming off the Pill is definitely a thing.

Despite what you may have been told when you were first prescribed birth control pills, they don’t cure the underlying reasons for your hormonal acne. Birth control throws a cover over them – nothing to see here! If you haven’t addressed the root cause, things are likely to return to how they were before the Pill.

The story usually goes something like this:

You stop the Pill. After a month or two, you get your period back. Your skin is looking good. Phew!

You hit the three-month mark. There are a few tiny breakouts; no need to worry; it’s all mostly good. Maybe the reports were exaggerated?

At four to six months – Noooo! The breakouts are back, perhaps worse than ever.
At this point, many women turn back to hormonal contraception thinking it’s their only solution for clear skin.

It’s not. But, what can you do?

If you suffer from acne, there's a very high chance you want to get rid of it - STAT! Isotretinoin, also often referred to by the brand names Accutane or Roaccutane, has a reported success rate of between 70-90% reduction in acne lesions.

Something so effective is probably pretty powerful stuff, and so it is with isotretinoin. Along with that power also comes side effects worth knowing about. These include some common, some less common and some contentious.

FAQ

Will I have to go on a restricted diet to get clear skin?

This is a common fear that many prospective clients have when deciding to work with a Naturopath on their acne.

My short answer is No; I am generally not a fan of restrictive diets, especially when applied as a blanket approach. You’ve got acne, and you can NEVER have dairy, gluten or chocolate again! Yikes – Ruuuun, scroll on!

What I DO champion is to help my clients discover a way of eating that makes them feel fabulous both inside and out. An inclusive way of eating that feels good. Food should not stress you out!

Sure, there are some health conditions, like coeliac disease, where you need to avoid gluten. These specific cases aside, restrictive eating patterns usually don’t work long-term because they are challenging to maintain.

Short-term we made need TEMPORARILY to eliminate some things while we work on digestive health, but in my mind, it’s never a forever plan.

This doesn’t mean you can eat whatever the hell you want. The focus of any eating plan for optimal health and well-being needs a nutrient-dense diet that fits your lifestyle and budget.

The list of possible rosacea triggers is a mile long and attempting to avoid ALL of them will leave you frustrated and disheartened about how hard it all is.

Not everyone with rosacea will be triggered by everything on this list. That’s why it’s important to discover your unique triggers so you can avoid them and not feel stressed about limiting so many things.

I encourage my clients to do a month long “rosacea diary” so we get detailed insights into what is going on for them. This then forms part of a holistic plan to support skin health.

Is it topical steroid withdrawal (TSW) or just an eczema flare?

Topical steroids are the first-line treatment for many skin conditions including eczema and atopic dermatitis. However, when stopping them (especially after longer term use) some people may experience a condition known as topical withdrawal syndrome (TSW)

TSW can be difficult to distinguish from a flare of the underlying dermatitis but a 2025 study provides some guiding criteria which can be helpful to clinicians.

It’s TSW if it’s:
✅ eczematous
AND includes:

✅ burning, flushing, OR thermodysregulation
AND 3️ of the following:

✅ bone deep itch
✅ profuse peeling
✅ "red sleeves"
✅ loose skin
✅ hair loss
✅ neuralgia/"zingers"
✅ lymphadenopathy
✅ swelling/oedema, or
✅ dry eyes

Why does it happen? Topical steroid, especially chronic use reduces collagen and elastin synthesis causing degradation of the dermal matrix. Corticosteroids also suppress nitric oxide in the skin (which is a vasodilator). Stopping them leads to a rebound increase in nitric oxide and this combined with the reduced support structure for the blood vessels just under the skin causes them to dilate and you get redness, swelling burning and flushing.

Interestingly, symptoms of TSW may appear in areas NOT exposed to topical steroids.

Why? Maybe because people with TSW have higher circulating levels of nicotinamide (NAD+).

So, how do you treat it?

Berberine containing herbs like Oregon grape root, and gotu kola can be helpful. Topical B12 and aloe vera have also shown clinical benefit to contain flares. Long term treatments focus on improving the skin barrier and balancing the skin microbiome.

29/09/2026

Like many chronic inflammatory skin conditions (psoriasis, atopic dermatitis) there is an increasing research and clinical interest in how the gut and skin microbiome interacts in the development and maintenance of HS lesions.

👉🏻 Why are we interested in supporting the microbiome in HS?

Dysbiosis in the microbiome drives systemic inflammation and immune dysregulation which are two key drivers of HS.

👉🏻 What are we learning?

There seems to be no one distinct type of HS dysbiosis but rather a change in the microbiome’s functional patterns with:
• Decreases in Cutibacterium spp. activity which affects skin pH and creates an environment for expansion of Staphylococcus aureus
• Reduction in retinol pathways
• Shift to biofilm formation which are more resistant to antibiotics
• Lower butyrate producers in the gut microbiome which impact inflammation
• Enriched pathways of D-glucarate and D-galactarate degradation which are associated with inflammatory gut disorder
• Increase in gram negative bacteria which impacts intestinal permeability, immune and inflammatory pathways.

Key messages:

🦠The gut/skin microbiome in HS seems to be more about the function of microbes (eg: SCFA production and biofilm formation) and less about what the individual microbes are.

🦠Think of the microbiome as an ecosystem which is influenced by external and environmental factors including: geography, diet, physical activity, medications, etc.

Clinical implications
🌿 Supporting the gut/skin microbiome in HS is more complex than just taking a “HS probiotic”! 💊

🌿 Targeting the microbiome offers an “upstream” approach to managing inflammation and immune imbalances that drive the lesions. There’s also evidence to show this can improve the action of biologic drugs.

🌿 Microbiome modulation in HS benefits from a nuanced, holistic and individualised approach, working alongside standard medical care. This is where naturopathy shines! 😊

24/09/2026

A 2021 study of 114 patients with chronic plaque psoriasis looked at the differences in disease activity, severity of gastrointestinal symptoms and the blood concentrations of the bacterial metabolite TMAO in those with normal intestinal barrier function again and those altered function.

The results showed that those with increased intestinal permeability has higher scores on the PASI (psoriasis area severity index) of almost double that of those with normal intestinal permeability. The high intestinal permeability group also had increased gastrointestinal symptoms particularly pain and bloating, increased inflammatory markers and higher TMAO levels. TMAO is an intestinal bacterial metabolite which can activate inflammatory pathways and alter immune function. The authors hypothesised that the TMAO translocation from the gut to the blood links gut dysbiosis to immune activation.

So what are my clinical takeaways from this research? The increasing importance of addressing the gut-skin connection in holistic psoriasis treatments. This includes promoting a healthy microbiome and regulating intestinal barrier function.

Intestinal barrier function can be optimised by doing the following:
• Increasing plant fibre
• Increasing plant phytochemicals like polyphenols and quercetin
• Reducing saturated fat consumption
• Reducing refined sugar
• Reducing alcohol consumption, avoid binge drinking

While this might not be all you need to do to manage psoriasis, implementing these tips can be helpful for reducing symptoms and improving your quality of life.

22/09/2026

A growing body of research supports the value of addressing digestive health and microbiome dysbiosis in skin conditions, including atopic dermatitis, psoriasis, rosacea, acne vulgaris, and hidradenitis suppurativa.

Central to this are the modifiable factors of:
· Diet
· Chronic stressors
· Sleep habits and quality
· Physical activity
· Smoking and alcohol consumption
· Antibiotic and other medication use

This is where the whole-systems, holistic approach of Naturopathy, alongside standard medical care, can enhance dermatologists' prescribed therapies.

It’s so much more than a simple prescription of a generic “skin” probiotic and a handout of the “Top 10 foods for [insert skin condition]”.

My aim as a clinical naturopath is to identify the areas most relevant to the individual microbiome, with consideration for personal and cultural preferences, budgets and other barriers and challenges. Education and resources are targeted rather than generic, with practical implementation paced appropriately so you don’t get overwhelmed.

I enjoy collaborating with other health care professionals as part of a team rather than being someone with “all the answers”. I have seen the benefits of this approach for patients who have experienced improved symptoms, quality of life and longer remission and, at the end of the day – that’s what matters most.💚

The authors of a 2026 review paper, “Latest and Greatest in Inflammatory Skin Disease and Gut Microbiome,” argue that dermatological care must evolve toward a more integrated, system-based model, and naturopaths have a lot to offer in this space.

Chronic low- grade stress elevates the hormone cortisol. Cortisol in turn raises insulin and this has a knock-on effect to your other hormones particularly the androgens and progesterone.

While it’s not practical to quit your job and head to a hammock by the beach, a daily relaxation wind down or self-care practice is an essential part of acne management.
