Ascites โ what is it?
Ascites is the abnormal accumulation of fluid inside the peritoneal cavity.
A common cause is cirrhosis, where portal hypertension, reduced effective circulating volume, sodium/water retention, and lower oncotic forces all contribute to fluid collecting in the abdomen.
A simple way to remember it:
Portal hypertension + fluid retention โ ascites
Common clues
* increasing abdominal swelling
* rapid weight gain
* abdominal discomfort
* fullness / early satiety
* shortness of breath when severe
Common causes
* cirrhosis (very common)
* malignancy
* heart failure
* kidney disease
* infection, depending on the setting
Why it matters
Ascites may indicate significant underlying disease, and complications can occur, including:
* breathing difficulty from abdominal distension
* discomfort and reduced mobility
* infection of ascitic fluid such as spontaneous bacterial peritonitis
Evaluation and management
Assessment may involve:
* physical examination
* ultrasound
* diagnostic paracentesis in appropriate cases
Management depends on the cause, but often includes:
* treating the underlying condition
* sodium restriction
* diuretics
* paracentesis when needed
๐จ Fever, abdominal tenderness, confusion, or worsening illness in a patient with ascites should prompt urgent medical review.
Easy memory tip
Ascites = fluid in the abdomen
Cirrhosis = common cause
