Wondering what the FTH GOALS Dementia Journey Program is?

A Dementia Care Program; created From The Heart

Not the journey most families ever plan (or wish) to embark upon, the dementia journey itself CAN be relatively smooth and calm with light pockets of turbulence shifting families in their seats, OR it can be extreme; tossing families violently about: leaving them clinging madly into thin air. The difference in the journey is in attitude, expectations and a healthy dose of preparation founded on education.

As with any journey; once we decide on a destination, we begin creating an itinerary and setting PRACTICAL goals in order to help initiate that itinerary (note the emphasis on practicality. You may want to go deep sea fishing on your journey, but is that a practical goal if your journey is to the mountains of West Virginia)?

Practical goal setting not only creates realistic expectations, it helps minimize stress levels and allows us the freedom to take on a relaxed attitude if and when our day doesn't go quite as planned (and where dementia is concerned, an easy going attitude will become your strongest asset).

Practical goals for any upcoming journey revolve around what we want to do while there; places we want to visit; people we want to see. Essentially, the EXPERIENCES we want to create. The journey through dementia is no different.

From The Heart Supportive Care's dementia journey program; GOALS was created to assist and support families during their personal journey through dementia. The first of its kind, GOALS is here to guide and support families living with dementia through community referrals and resources, accessing available funding for care, home assistance and safety equipment, up to date education and insight, patient advocacy, effective behavior techniques, identifying persona glimmers, as well asl triggers, creating both family managed and UCP care plans, and where applicable; sourcing professional personal care. Much like a tour guide in a foreign country, your FTH team is there from the first day of diagnosis to walk you through planning, goal setting, and every question, stage, concern, and transition.

Happy Saturday, everyone!

With dementia numbers on the rise, we would like take a break from discussing dementia CARE, and shift our focus temporarily on dementia PREVENTION. Specifically...diet and establishing healthy eating habits.

***The MIND Diet, aka Mediterranean DASH-diet Intervention for Neurodegenerative Delay, is a combination of two well-established healthy diets. One is the Mediterranean Diet which contributes to reducing the risk of obesity, diabetes, cardiovascular disease, and cancer. The other is the DASH diet, Dietary Approaches to Stop Hypertension, which is effective in reducing high blood pressure.

It just so happens that in addition to addressing the aforementioned health concerns, these two diets also show promise in preventing cognitive decline. Researchers at Rush University and Harvard decided to put them together and see what would happen.

The results of two large, multi-year studies revealed that the MIND diet is associated with significantly slower cognitive decline and a 50% reduction in Alzheimer’s disease risk.

Even a little adherence to the diet is beneficial. One of the studies showed that while closest adherence to the diet resulted in a 53% decreased risk of developing Alzheimer’s, moderate adherence still showed a 35% reduced risk.

**Our Dementia Care Consulting/Coordinating program has been very well received this season. This program was created as a means to offer families an alternative to traditional agency home care for those living with a dementia/Alzheimer's diagnosis.
Available for every stage of dementia, ideally our Dementia Consulting and Care Coordinating Programs would begin at initial diagnosis (no worries; we do welcome every stage of progression) and accompany families throughout their complete dementia journey.
Through offering educated options unique to each type of dementia, personalized direction and guidance , Client and family support/assistance, advocacy and physical/emotional and Pastoral care; our personalized, revolutionary program is the first of its kind.

Wondering why you would need a Dementia Care Consultant & Coordinator?

While at home personal care is a standard scope of practice for PSWs, personal care for DEMENTIA is very different. Typical methods of providing personal care is not usually welcome (or permitted) by those living with dementia, and more often than not leads to aggressive/violent behaviours caused by the fight or flight response.

This not only creates a lack of consistant care being provided, it creates barriers to receiving and retaining personal care /support from outside sources.

While we, as Personal Support Workers have a diverse knowledge of providing safe, effective personal & supportive home care...Dementia Care Consulting is a combination of SPECIALIZED personal supportive care, up to date education and resources, health care advocacy,
behaviour tips /techniques, master communication skills, mental health first aid, and foresight for avoidance of common triggers. A DCC is the profession of choice for personal and supportive care in dementia.

Your family physician has a general knowledge of health issues, however a diagnosis always results in referal to a SPECIALIST. A Dementia diagnosis is no different, and requires HIGHLY specialized care.
