Hamilton Urban Core Community Health Centre

Hamilton Urban Core Community Health Centre As one of Ontario’s 76 Community Health Centres, Hamilton Urban Core is non-profit, charitable and community-governed, and provides multi-disciplinary inter-professional health care framed by the social determinants of health.

Hamilton Urban Core Community Health Centre is a community-based agency mandated to provide primary health care services. We believe that Hamilton Urban Core Community Health Centre is integral to the community in which we exist and serve. We believe that peace, shelter, food, income, a stable eco-system, sustainable resources, social justice and equity are fundamental conditions to the health and well-being of individuals and communities. We recognize that a healthy community can only be achieved through collective action. Our vision for Hamilton Urban Core Community Health Centre is:

•of a centre where people have equitable access to health and well-being, as it is free of systemic barriers
•that people are in control of their health and destiny, individually and collectively
•of an environment in which people are treated with respect and dignity
•of belonging to a healthy community in which there is collective responsibility to take care of one another and the environment
•of being an agent of change which influences and challenges social, economic and political structures and policies to achieve access, equity and equality in primary health care.

Hamilton Urban Core published Health Services for Syrian Refugees Response Report

Syrian individuals and families began arriving in Hamilton on December 21, 2015 under the Government Assisted Refugee Program. Hamilton Urban Core was engaged in early planning with the local reception centre, Wesley Urban Ministries, who was also responsible for the federally funded Refugee Assistance Program (RAP). The Minister of Health and Long-Term Care requested that community health centres across the province be engaged in leadership roles to ensure that the Syrian refugees had access to health services. Through the Hamilton health services planning table Hamilton Urban Core along with other health providers had early involvement in connecting with people and providing information, conducting initial health assessments and responding to urgent situations. This report contains information about the approach and services provided to refugees from Syria who accessed services at Hamilton Urban Core

Hi, im just trying to see if you can help me find out some info. I have a friend whom is a male. He just left a relationship where the girlfriend was hitting him as well as she is having strangers jump him and rob hin... some of these people are staying in the shelters. So not only is he afraid of catching covid and sleeping on the streets now hes stuck worrying that he might get jumped at any second by numerous people. Someone told me that there is a program for the homeless in hamilton that would put him up in a hotel/motel . Do you have the info for this program?? Thank you
Your site is a mismanaged utter failure. That’s poisoned the surrounding area with unsanitary conditions, open drug dealing/use, non stop loitering, yelling and screaming and trash and waste being thrown absolutely everywhere. You should not be operating if you are unable to control your environment, which clearly you lost control of sometime ago. Shame.
Please join us at this event in Hamilton - lived experience speakers talking about social justice and change!
Hmmm it must have been a narcissist that turned away Men's Street Ministries last week?! Why on God's earth would you turn away a Christian ministry- one with a pure heart - who was handing out (free) food, clothing and hope to the homeless on your property? I'd love to hear your answer
