Wolf Creek PCN Welcome to the Wolf Creek PCN! We are part of your Patient Medical Home! PCNs develop solutions to meet the needs of the local community and their health issues.

Primary Care Networks (PCNs) bring local physicians and other health care professionals together to provide comprehensive patient care to Albertans. PCNs are comprised of groups of family physicians working with other health care professionals such as nurse practitioners, nurses, dietitians, pharmacists, social workers, exercise specialist, behavioral health consultants, and mental health professionals. PCNs are created through an agreement between physicians and Alberta Health Services. Please note that PCNs are not a governing body to physicians; the College of Physicians & Surgeons of Alberta regulates the practice of medicine in Alberta. There are 40 PCNs operating throughout Alberta with more than 3,800 family physicians, and more than 1,400 other health practitioners. At any given time, there are hundreds of PCN family doctors across Alberta accepting new patients. Research shows that patients who regularly visit a family doctor are healthier and live longer. The Wolf Creek PCN supports the Central Alberta communities of Bentley, Delburne, Innisfail, Lacombe, Ponoka, Rimbey, and Sylvan Lake. Less daylight means it’s time to think about visibility and safety. With darker mornings and earlier sunsets, take a few extra precautions when you’re walking, driving or heading out for the day.

🚶 For pedestrians: Wear bright or reflective clothing, stay visible and use marked crosswalks. Make eye contact with drivers before crossing.

🚗 For drivers: Slow down in low-light conditions, watch carefully for pedestrians and cyclists, and be especially alert in school zones and at crosswalks.

❤️ For families: Talk to kids about staying visible, using designated crossings and paying attention around roads and school drop-off areas.

A few simple changes can help everyone get where they’re going safely this fall!

This weekend is a time to gather, reflect, and give thanks to others, but remember to make space to support your own mental health too.

Taking care of your mental health might look like:

🤍Setting boundaries to help you from feeling overwhelmed
🤍Recognizing when you need to take a break and having a plan to help you recharge

Your mental health matters to us, too. For help with strategies, tools, and resources to support your mental health, talk to your primary care provider.

Remember, you do not have to wait until your mental health concerns feel overwhelming to ask for help. Ask for help any time to get the support you need to feel your best.

Wet leaves, slippery sidewalks and changing weather can make getting around a little trickier this time of year! 🍂

Help prevent slips and falls by:
👟 Choosing shoes or boots with good traction and a secure fit
👟 Using well-lit paths and carrying a light when needed
👟 Taking extra care on stairs and using handrails
👟 Watching for wet leaves, uneven surfaces and slippery spots

September 30 is the National Day for Truth and Reconciliation. 🧡

For our PCN, reconciliation means continuing to listen, learn and take meaningful action to create more welcoming, culturally safe and accessible health-care experiences for First Nations, Inuit and Métis peoples.

There are many ways to mark the day, from attending community gatherings and commemorative events to listening to Indigenous voices and learning more about the history and ongoing legacy of residential schools.

As school, work and activities get back into full swing, it's a good time to get back to the healthy habits that help protect you and those around you.

🧼 Wash your hands regularly
🤧 Cover coughs and sneezes
🏠 Stay home when you're sick
🧹 Clean frequently touched surfaces

Healthy habits don't have to be complicated. Small steps can help reduce the spread of illness wherever your routine takes you.

Watch for symptoms and reach out to your primary care provider or PCN team if you need support
