September was back to school for the Strive team too and 3 of our team members are pursuing post graduate degrees.🤓

Patrick is taking a 1 year Advanced Health Care Practice Master of Clinic Science (MClSc) at Western University. It involves 3, 2-week residency periods between Sept-July, otherwise he will be in clinic 3 days a week starting this fall.

Melanie is in year 2 of her PhD in Health Professional Education Research (HPER) at the Wilson Centre which is an extra-departmental unit in the Temerty Faculty of Medicine and a collaboration between the University Health Network and the University of Toronto. Melanie continues to be in clinic 2.5days a week.

Nicole is in year 3 of her PhD in Rehabilitation Science, with a Collaborative Specialization in Neuroscience at the University of Toronto. Her doctoral research focuses on understanding the importance of assessing and treating walking adaptability following spinal cord injury. Nicole continues to offer 2 days a week in clinic.

At Strive, we believe in supporting our clinicians with their personal and professional goals. There is a powerful synergy between knowledge gained from clinical, educational and research realms. September is back to school month and the many of the Strive team members are also heading back to school. Last week, Strive Physio Grace Underwood spent 2 days at . She was there to assist Strive alumni Physio Kiah to screen all the students to ensure they start off on the right foot! This annual screening helps to identify areas of weakness, lack of or excess mobility and helps inform injury prevention and training. Thanks to Kiah for the invite and to Grace for offering her time and expertise to support these dancers 2 years in a row!

Time to shine the light 🔦 on Milena!

Milena began practicing Pilates in Montreal in 2001. Her interest in the method led her to acquire her certification with Polestar Pilates in 2008. Since then she has taught in various studios, health and fitness centres and corporate facilities. She has participated in workshops with distinguished instructors Brent Anderson, President and CEO of Polestar Pilates and Pilates Elder, Lo**ta San Miguel. Aside from continuing her Pilates education and practice, Milena is a visual artist and obtained her MFA from York University in 2014.

'Every instruction we give, every course of action we set, every result we desire, starts with the same thing: a decision' (Start with Why p.15). Our GLA:D team can't wait to meet you! If you have hip and knee osteoarthritis and struggling with pain and lack of mobility and overall function, then this program may be for you. What makes this program unique is that is starts with a Physiotherapy assessment to determine you suitability. After that, the physio will create an individualized exercise program that considers your unique exercise needs. You then get to work out in a group setting, creating accountability and building motivation, while you follow your individualized program.

These sessions are led by Grace, Alex or Stephanie and they will be keeping a watchful eye on your form, tracking your progress and changing the exercises as you get stronger.

At Strive, the GLA:D™ Canada program is delivered in a hybrid model, offering both in-person and virtual opportunities to participate. To determine if you will benefit from the GLA:D™ Canada program you will have an assessment by a Strive Physiotherapist, which will include a variety of mobility, strength and movement measures to determine if this is the right program for you.

How long is the program?

The GLA:D™ Canada program consists of:
✅ 2 education sessions which teaches participants about OA, including the degeneration process in the joint, how the GLA:D™ Canada program exercises improve joint stability, and how to retain this improved joint stability through day to day self-management techniques.

✅ 12 Neuromuscular exercise sessions (x2 per week for 6 weeks) to improve muscle control and stability of the joint, which leads to reduction in symptoms and improved quality of life. Participants are educated on how to include the neuromuscular exercises in daily physical activity and advised to continue to be physically active to ensure that the improvements in muscle stabilization are maintained in the long term.

Who Is Eligible For The GLA:D™ Canada Program?

Anyone with pain or stiffness from knee and/or hip OA. This includes mild to severe disease. X-rays or a Doctors referrals are not required for participation in the GLA:D™ Canada program.

Another terrific review coming your way...this time we are spotlighting Nicole Cesca! 🔦

Originally from Windsor, ON, Nicole attended the University of Windsor where she completed her Honours Bachelor of Science in Biochemistry. During her time at the University of Windsor, Nicole was also a member of the Women’s Varsity Golf Team. 🏌️‍♀️

She then went on to complete her Master of Science in Physical Therapy at the University of Toronto. During her physiotherapy education, Nicole enjoyed clinical internships in spinal cord injury (SCI) rehabilitation, paediatrics, and orthopaedic physiotherapy. Nicole is dedicated to being a lifelong learner and is currently completing her PhD👩🏼‍🎓 in Rehabilitation Science with a Collaborative Specialization in Neuroscience at the University of Toronto.

Nicole is passionate about neurological and orthopaedic physiotherapy and plans to continue to advance her practice by pursuing courses in manual therapy, integrated dry needling, and acupuncture.

Nicole’s love for golf has led her to continue advancing her knowledge of connection between the body and golf swing. BPPV is characterized by brief episodes of vertigo triggered by specific movements. Some common triggers of BPPV are:
1. Rolling Over in Bed: Changing positions while lying down can displace the inner ear crystals, leading to vertigo
2. Looking Up or Down: Tilting your head up or down can disturb the balance organs and trigger BPPV.
3. Changing positions : Getting up from lying down or transitioning from sitting to lying down, or even leaning forward, can cause sudden movements that exacerbate symptoms.

How Does Physiotherapy Help?
1. Skilled Diagnosis and Assessment:
- Find a physiotherapist, like our very own Nicole Cesca, with expertise in the assessment and management of vestibular conditions, including BPPV.
- At Strive, Nicole Cesca uses specialized infrared video goggles, also known as infrared video oculography, as part of her comprehensive vestibular assessments. These goggles allow Nicole to closely observe your eye movements, significantly enhancing the accuracy of her evaluations and informing the management plan.

2. Symptom Relief With Repositioning Techniques:
- Nicole may also use specific techniques, such as the Epley or Semont manoeuvres, to reposition the otoliths and alleviate vertigo and dizziness. Repositioning helps the particles resorb more easily, reducing uncomfortable symptoms.

3. Education and Exercise:
- You will learn more about BPPV so you can better understand its triggers and how you can manage the symptoms at home. By minimizing symptoms, you can regain independence and improve your quality of life.
- Your management plan is likely to also include individualized exercises to improve balance, strengthen neck muscles, and enhance head movement control.

And here's a little more about Grace:

Growing up Grace was an avid athlete competing in and coaching soccer and volleyball.⚽️ 🏐 In addition to those sports, growing up Grace was formally trained in ballet, pointe, and jazz.🩰 As her passion for sport and dance continues, she has taken courses exploring dancer’s physiotherapy and athletic taping. She is passionate about helping athletes recover and prevent injuries to keep them participating in the activities they love.

In addition to working at Strive, Grace also works at St. Joseph’s Health Centre.🏥 There she works with patient and orthopaedic surgeons treating patient after joint replacements, fractures, and various other orthopaedic surgeries such as meniscectomies and rotator cuff repairs.

Grace believes in a holistic patient-centred approach to treatment that combines manual therapy, exercise prescription, and education. She enjoys working with people of all ages and aims to work collaboratively with her patients to help them achieve their goals.

Outside of the clinic Grace enjoys furthering her passion with sports as she coaches and play volleyball in the west end of Toronto as well as going to the gym and spending time with her family and dog.

What Is ‘Snapping Hip’ Syndrome? 🫰🏼

‘Snapping Hip’ Syndrome occurs when a dancer experiences popping, snapping, or cracking type sensations at the front or side of their hip. It typically occurs when bringing the leg into a developpé devant or à la seconde. This snapping of the hip is often pain-free at first, but over time, the tendon gets inflamed and starts to limit dance participation.

What Causes ‘Snapping Hip’ Syndrome? What Are Common Symptoms? 🙋🏻‍♀️

‘Snapping Hip’ Syndrome is caused by the rubbing of one of the hip tendons over the bony landmarks of the hip or pelvic bone during movement. The most common muscles that cause this snapping sensation are the Iliopsoas tendon and the Tensor Fascia Latae. This often occurs when there is an imbalance between the amount of turnout a dancer is using and the ability of the hip muscles to control that turnout range. As the dancer moves the hip into an extension, if the muscles cannot maintain the turnout they had when the feet were on the ground, they often lose that turnout as the leg moves higher, and this can be when the ‘snap’ occurs.

Common symptoms include:
- Musical noises in the hip when moving through extensions
- This may or may not be painful
- Pinching or pain at front of hip when it is fully flexed, with or without internal rotation of the hip

How Can Physiotherapy Help My ‘Snapping Hip’ Syndrome?

Strive Physiotherapist and lead of our Dancer Care program of care, Grace Underwood has been in your shoes. She combines her dance experience and physiotherapy knowledge and skill to perform a detailed assessment to determine which muscle is the cause of your snapping and how irritable (easily aggravated) it is. A Pre-Season Screen is a physical assessment to determine readiness to return to dance. Your Strive Physiotherapist Grace Underwood, will look at your dance technique, mobility, posture, and strength, and ask you about pain and any injuries you may have experienced.
Screens are important for injury prevention and identifying any factors that have the potential to contribute to re-injury or injuries that may occur in the future. After the assessment, your physiotherapist will help you to create a plan to improve any risk factors that may have been picked up.

What Is Achilles Tendinopathy?

The Achilles tendon is the tendon that attaches the calf muscles to the heel. Pain in this tendon is called Achilles tendinopathy. The Achilles tendon is a vital part of transmitting load and force up from the foot and ankle to the rest of the lower extremity and it serves dual purpose to absorb shock from the lower extremity.

What Causes Achilles Tendinopathy? What Are Common Symptoms?

Achilles tendinopathy often occurs when the foot and ankle experience excessive loading, such as a sudden increase in training volume or a change in activities that require the tendon to work differently then it is used to. This can occur seemingly ‘out of the blue’ or suddenly during an activity that requires an explosive effort.

Common symptoms include:
Pain to touch or squeeze the Achilles tendon
Swelling and thickening in the area of pain

How Can Physiotherapy Help My Achilles Tendinopathy?
Your Strive Physiotherapist can help to treat both the direct causes of your pain, as well as contributing factors to why it developed in the first place. Your physiotherapist will employ manual therapies such as massage to the muscles of your calf and joint mobilizations to the bones in the foot and ankle, and potentially modalities such as Shockwave therapy.

You will be given a tailored exercise program to help strengthen the muscles contributing to your symptoms and get you back to your regular activity. Your Strive Physiotherapist will help you find the right balance of challenging the Achilles tendon with work to promote healing without overdoing it. If your pain is due to a running injury, a gait and running analysis can be done and advice given on technique, and training errors.
