🩺 Powering connections to preventive health.

Yesterday, community members came together for a session on cancer screening and the importance of primary care as part of our Health & Wellness Series (55+).

Access to trusted health information and regular primary care can help people take an active role in their health, including staying up to date with preventive care and screening.

This Community Health & Wellbeing Month, we’re continuing to Further Health Equity and Power Connections by creating opportunities for our community to learn, ask questions, and connect to health information and care.

Thank you to everyone who joined us! 💚

Powering connections to primary care.

This Community Health & Wellbeing Month, we’re highlighting an important part of a healthier community: having access to a regular primary care provider.

Having a primary care provider can help you stay connected to ongoing care, preventive services, and the health supports you need over time.

If you live in the M3C and M2J communities and need a family doctor or nurse practitioner, we can help connect you to care.

This Community Health & Wellbeing Month, let’s get moving!

Our Low Impact Aerobic (55+) program is a great way to stay active from home while building strength and supporting overall health and wellbeing.

Each virtual session includes a warm-up, cardio exercise, strength training, and cool down, all designed for participants who can sit or stand without assistance.

By creating accessible opportunities to stay active and connected to health, we’re bringing this year’s theme, Furthering Health Equity, Powering Connections, to life.

Just ahead of Community Health & Wellbeing Month 2026, our Community Health Ambassadors joined the Parma Court BBQ, organized by the TCH Violence Reduction Program, to connect with community members and share information about health services and supports available through FHC.

Our team shared resources on primary care, cancer screening, chronic disease management, and FHC programs while connecting directly with community members.

This is what Furthering Health Equity, Powering Connections looks like in action: meeting people in their communities, sharing accessible health information, and strengthening connections to care and support.
