✅ CLINICAL CASE: THE ANSWER

The answer is A: 4.0 mm × 10 mm.

Why? 👇
🔹 Length: 13 mm minus a 2 mm safety zone leaves 11 mm. A 10 mm implant fits.
🔹 Width: 7.5 mm ridge minus 4 mm implant leaves 1.75 mm of bone on each side.
🔹 Space: 4 mm implant plus 1.5 mm on each side needs 7 mm. You have 9 mm.

The take-home: Height minus 2 mm. Width minus 3 to 4 mm. Space minus 3 mm.

✅ CLINICAL CASE: THE ANSWER

The answer is C: Evolving vascular occlusion: see her now.

Why? 👇
🔹 A net-like dusky pattern, pain that is getting worse, slow capillary refill and cool skin, in the territory of the facial and angular artery.
🔹 Pustules are a late sign of ischaemia, often mistaken for herpes or acne.
🔹 This is a time-critical emergency.

The take-home: Any “bruise” with increasing pain after filler is vascular until proven otherwise.
