# # **History is the Foundation of Allergy Diagnosis**
A 38-year-old lady presented with severe chronic urticaria, along with allergic rhinitis and allergic conjunctivitis. She was extremely frustrated by persistent itching and requested an allergy workup to identify the cause.
As always, I began with a detailed history. Initially, she was reluctant to answer several questions, wondering how they were related to her allergy. However, after carefully reviewing her previous medical records, an important clue emerged—she had a history of fluctuating thyroid disease, alternating between hyperthyroidism and hypothyroidism over the years.
This raised the suspicion of autoimmune thyroid disease. Further investigations confirmed positive anti-TPO and TSH receptor antibodies, establishing the diagnosis of autoimmune thyroid disease.
The allergy evaluation, combined with careful clinical correlation, showed that the long list of foods she had been avoiding unnecessarily did not need to be restricted. She was reassured, counselled appropriately, and her treatment was directed towards the underlying autoimmune association rather than attributing the condition to foods without evidence.
**Take-home message:**
🔹 In allergy practice, history is the foundation that guides the right investigations and their interpretation.
🔹 Chronic urticaria may have an autoimmune association, including autoimmune thyroid disease, and this possibility should always be considered.
🔹 A meticulous history, combined with appropriate investigations, prevents unnecessary food restrictions, reduces patient anxiety, and leads to better clinical decisions.
