Identifying the root cause of piles is important.

In this episode Dr. Arim Khan โ€” MBBS,
MD Microbiology (resident), background in psychology,
advanced training in gut health and the gut microbiome
is joined by Dr. Ismail Khan โ€” MS (Surgery), MRCS
(England), Minimal Access Robotic and Bariatric
Surgery. He operates on gallbladders, piles, fissures,
hiatus hernias and obesity every week, but this entire
hour stayed on one question: which of these problems
resolve with lifestyle, which need proper medical
evaluation, and when is surgery genuinely necessary.

We also covered where the myth of a lifelong fat-free
diet after gallbladder removal came from, why the same
stone measures differently on two ultrasounds, what's
being sold as "laser" treatment for piles, and the red
flags people ignore for years because they assume it's
just gas.

If you've been dealing with bloating, constipation,
acidity or a fissure that keeps coming back, this one is
for you.

Does your age only depend on yearsโ€ฆ or do your gut bacteria get a say too? ๐Ÿฆ โณ

In 2022, scientists at the Quadram Institute in the UK published a study in the journal Microbiome. They swapped the gut bacteria of young and old mice using a f***l microbiota transplant. What they saw just 18 days later was surprising ๐Ÿ‘‡

๐Ÿ”„ Old mice + young bacteria:
- Brain inflammation went down
- Eye inflammation dropped to young levels
- RPE65, a protein essential for vision, was restored to young levels
- Signs of gut "leakage" into the blood went down

โš ๏ธ Young mice + old bacteria:
- Inflammation rose in the brain and eyes
- The same vision protein dropped
- The gut barrier showed signs of becoming leaky

๐Ÿ“Œ KEY TAKEAWAYS
1๏ธโƒฃ Gut bacteria do more than digest food. They can affect the health of your brain and even your eyes.
2๏ธโƒฃ Your gut lining is just one cell thick. It's your body's "Border Wall."
3๏ธโƒฃ When that wall weakens, bacterial toxins reach the blood and keep the body in a state of chronic inflammation. Scientists call this Inflammaging.
4๏ธโƒฃ Fiber is food for your gut bacteria. In a separate study (Cell, 2016, also in mice), bacteria deprived of fiber started eating the gut's own protective mucus layer.
5๏ธโƒฃ Add more fiber to your daily plate: dal, chickpeas, rajma, vegetables, fruits, millets and whole grains.

๐Ÿ”ฌ To be honest: this research was done in mice, not humans. Memory tests also showed no difference. Don't treat it as an "anti-aging treatment," and stay away from any "young bacteria" products or DIY transplants.

Next time you eat, ask yourself: "What is my plate feeding my gut bacteria?"

๐Ÿ‘‰ Follow for the next video, where I'll share 5 everyday habits that are breaking down your gut barrier.

Constipation hai to fibre ki quantity ke saath uski variety bhi dekhiye

Apni tolerance ke hisaab se oats kiwi cooked vegetables aur dal ki chhoti portions se shuru karein. Ek naya source add karein aur response dekh kar dheere badhayein. Sab kuch ek saath badhane se bloating badh jaati hai

Microbiology humein ye samjhati hai ki alag gut microbes alag substrates use karte hain. Fermentable fibres unke liye food hain jinse short chain fatty acids bante hain. Isliye sirf fibre ke grams nahi balki food sources ki diversity bhi important hai. Lekin diversity build karne ka matlab symptoms ke bawajood zabardasti khana nahi hai

Video mein maine PHGG aur psyllium husk ka naam liya hai. Dono soluble fibres hain lekin kaam bilkul identical nahi hai

PHGG fermentable fibre hai jo microbial activity ko support karta hai aur kuch clinical studies mein bloating mein benefit mila hai

Psyllium ya isabgol paani hold karke gel banata hai aur stool consistency improve karta hai. Constipation ke liye iska evidence better established hai. Har useful fibre ka main kaam microbes ko feed karna nahi hota

Is follow up mein pehle se chal rahe PHGG ke saath psyllium add kiya gaya. Ye individual response ke hisaab se strategy thi. Har person ko dono ki zaroorat nahi hoti. Doosre interventions bhi chal rahe the isliye poore improvement ka credit sirf in do fibres ko dena correct nahi hoga

Supplement use karein to low dose se start karein. Psyllium full glass paani ke saath lein aur dry powder na niglein. Dose tolerance ke hisaab se dheere badhayein

Lekin stool soft hone ke bawajood bahut zor lagana padta hai ya blockage jaisa lagta hai to sirf aur fibre add karna solution nahi hai. Pelvic floor coordination ka issue ho to uska assessment aur targeted treatment chahiye. Medicines thyroid problems aur slow transit mein bhi fibre alone enough nahi hota

Bleeding ko piles maan kar fibre badhate rehna bhi sahi approach nahi hai. Blood in stool ka assessment zaroori hai. Severe pain vomiting ya gas pass na hone par urgent medical care lein

Mere liye constipation ko samajhne ka matlab sirf kitni baar stool aaya nahi hai. Stool ki consistency straining incomplete evacuation aur symptoms ka p

Histamine bolte hi camera ki battery khatam ho gayi ๐Ÿ˜‚ Explanation ka baaki hissa yahan hai

Skin mein mast cells hote hain. Inki activation se histamine aur doosre mediators release hote hain jo urticaria ke itchy raised patches banate hain

Mera view hai ki fibre ko sirf pet saaf karne wali cheez samajhna uski biology ko underestimate karna hai. Kuch fibres stool mein paani hold karte hain. Kuch ko gut microbes ferment karke short chain fatty acids banate hain. Ye compounds gut lining aur immune signalling mein role nibhate hain

Maine kuch papers padhe hain jinmein chronic urticaria wale patients mein gut microbes aur unke metabolites mein differences mile hain. Ek study mein unke microbes mice mein transfer karne par gut permeability aur mast cell driven skin inflammation bhi badhi. Matlab gut aur skin ke beech biological communication bhi hota hai. Lekin mice ka result har insaan ka diagnosis nahi hai

Is person ne fibre lene ke baad hives aana band hone ki baat batayi. Ye observation hai. Isse ye prove nahi hota ki usko leaky gut tha ya fibre ne urticaria cure kar di

Constipation ke liye mera practical preference soluble fibre hai. Isabgol ke liye clinical evidence better established hai. Adult mein low starting dose jaise 2.5 gram daily se shuru karke tolerance ke hisaab se badhayein. Ek full glass paani ke saath lein aur dry powder kabhi na niglein

Bloating prominent ho to PHGG ek option hai jiske liye clinical trial data bhi hai. Low dose se shuru karein aur dheere badhayein. Dono supplements ek saath start karna zaroori nahi hai. Inulin ya wheat bran ki badi dose se shuru karna sensitive gut mein gas badha deta hai

Oats fruits vegetables aur tolerated dal se food fibre bhi build karein. Fibre ke saath sleep stress aur bowel routine ko address karna bhi zaroori hai

Fibre urticaria ki prescribed treatment ka replacement nahi hai. Apni medicines khud band na karein

Ye reel sirf health education ke liye hai. Personal diagnosis ya prescription nahi hai aur kisi cure ki guarantee nahi deti

Aapke liye zyada difficult kya hai constipation ya bloating

Calprotectin high aaya toh kya automatically IBD hai?

Calprotectin ek protein hai jo mainly neutrophils naam ke immune cells mein hota hai. Jab intestine mein inflammation hoti hai toh stool mein iska level badh sakta hai. Yeh inflammation ka clue hai. Exact disease ka naam nahi batata.

Crohnโ€™s disease aur ulcerative colitis mein yeh badh sakta hai. Lekin gut infections aur diverticulitis mein bhi. Kabhi untreated celiac disease aur microscopic colitis mein bhi elevated mil sakta hai. Kuch colorectal polyps aur cancers bhi ise badha sakte hain. Lekin yeh cancer screening test nahi hai.

Ab false positive samajhiye. Agar report high hai lekin IBD nahi hai toh zaroori nahi test galat tha. Infection ya medicine ki wajah se actual inflammation bhi ho sakti hai.

Ibuprofen aur diclofenac jaise painkillers intestine ki lining ko irritate kar sakte hain. Aspirin aur acidity ki PPI medicines ke saath bhi mild ya borderline elevation mil sakti hai. Isliye doctor ko apni medicines batana important hai. Khud se prescribed medicine band mat kijiye.

Sample collection aur laboratory methods se bhi result mein variation aa sakta hai. Borderline result aur bahut high result ko ek jaisa interpret nahi karte.

Is case mein childhood trauma aur anxiety ke saath do separate reports mein calprotectin high tha. Anxiety gut symptoms badha sakti hai. Lekin repeated high calprotectin ko sirf stress bolkar ignore nahi karna chahiye.

Exact values aur symptoms dekhkar gastroenterologist decide karte hain ki repeat test chahiye ya further investigation.

Report se panic mat kijiye. Lekin uska follow up zaroor kijiye.

What's ur childhood story.... If u ll comment in depth probably I might reply in reels ๐Ÿ˜Š
