The Department of Interventional Radiology, CMC Vellore, was proud to participate in the 21st Monsoon Meeting organised by the The India Neuro Interventional Foundation (INIF), a focused academic meeting dedicated to morbidity and mortality in neurointerventions.

Our department contributed three podium presentations, highlighting the importance of recognising, managing, and learning from complications in complex neurointerventional procedures:

🔹 Dr. Rajchandra kharayat, Consultant, Interventional Radiology, CMC Vellore presented -
• A complication and it’s management during Maxillary AVM Embolisation
•A complications and it’s management during High Cervical Dural AV Fistula Embolisation

🔹 Dr. Pareeksh*th Rai, 3rd Year DM Interventional Radiology Resident, CMC Vellore presented -
• A complication and it’s management during Stent-Assisted Coiling

Meetings dedicated to morbidity and mortality provide a unique platform for open academic discussion, reflection on challenging cases, and the exchange of practical experience. By sharing both successes and complications, we collectively strengthen patient safety, refine procedural strategies, and advance the field of neurointervention.

We congratulate our faculty and resident for representing CMC Vellore and contributing to this important scientific forum.

Making BRTO accessible: a cost-effective study from CMC Vellore

Balloon-occluded retrograde transvenous obliteration (BRTO) is a proven technique for gastric varices and hepatic encephalopathy — but dedicated occlusion balloons remain expensive and inaccessible in many centers. Our team studied whether low-cost, off-label balloons could safely bridge that gap.

The study: 31 patients (2009–2023) underwent BRTO using Swan-Ganz, Fogarty, or stone extractor balloons — selected based on shunt anatomy — with STS foam sclerosant.

Key findings:

1️⃣ Technical success in 93.5% of cases (29/31)

2️⃣ Clinical success in 92% of patients with follow-up (mean 953.6 days)

3️⃣ Complete variceal obliteration in 70.5%, partial in 29.5%

4️⃣ Balloon-related complications were rare and did not affect clinical outcomes — even the 2 cases of balloon rupture/deflation were managed without rebleeding

Why it matters: In resource-constrained settings, this offers a technically feasible, safe, and effective alternative to conventional BRTO — without compromising outcomes.

Prospective studies and standardized protocols are the next step.

How is your center approaching cost barriers in interventional radiology? We'd love to hear your experience with off-label device use.

The Department of Interventional Radiology, CMC Vellore was pleased to participate in the ISVIR TNPY Salem Chapter CME on Multidisciplinary Management of Portal Hypertension.

Prof. Dr. Shyamkumar Nidugala Keshava, Head of the Department, delivered a lecture on the stepwise management of hepatic venous outflow obstruction, emphasizing an anatomy-first approach to treatment planning.

The CME brought together IR, medical GI, and surgical GI specialists for a rich multidisciplinary discussion, with sessions covering TIPSS in cirrhosis, access strategies for gastric varices, and pediatric portal interventions post-transplant — anchored by a keynote from Prof. Dr. Mathew Cherian on integrating IR into multidisciplinary care.

The meet was also attended by residents from our department, who gained valuable exposure to regional academic discussions and networking within the field.

Congratulations to the organizing team and the ISVIR TNPY chapter for another well-run, enriching CME.

From precision imaging to precision therapy.

A 34-year-old woman with spinal dysraphism, tethered cord syndrome, and a history of multiple spinal surgeries presented with T10 neuropathic pain.

Following multidisciplinary evaluation, a CT-guided radiofrequency ablation (RFA) targeting the T9, T10, and T11 intercostal nerves was performed as a minimally invasive, image-guided pain intervention.

This case highlights the evolving role of Interventional Radiology in the management of complex neuropathic pain through targeted, image-guided procedures.

At CMC Vellore, we continue to advance patient care through innovation, precision, and multidisciplinary collaboration.

Sometimes, the most important diagnosis begins with asking the right questions.

A 62-year-old gentleman was referred to our Interventional Radiology service after an incidentally detected pulmonary arteriovenous malformation (PAVM) was identified during a routine health check-up. While he was asymptomatic from the pulmonary lesion, a detailed clinical history revealed recurrent episodes of nasal bleeding for the past 15 years along with a strong family history of similar complaints.

Recognizing the significance of these findings, he was evaluated through a multidisciplinary team involving Interventional Radiology, ENT, Pulmonary Medicine, and Medical Genetics. Comprehensive genetic evaluation identified a pathogenic ACVRL1 mutation, confirming an underlying hereditary vascular disorder and allowing us to plan his treatment with a holistic perspective.

After careful planning, he successfully underwent endovascular embolization of the pulmonary AVM using a vascular plug. The procedure was uneventful, and he recovered well with an excellent post-procedure outcome.

This case highlights several important lessons:

• Incidental findings should always be evaluated in the context of the patient's complete clinical history.

• Genetic evaluation can uncover an underlying diagnosis that has implications not only for the patient but also for family members who may benefit from screening and counseling.

• A multidisciplinary approach ensures comprehensive care and optimal decision-making.

• The right intervention, for the right patient, at the right time, can prevent serious complications while delivering minimally invasive, durable treatment.

At our Interventional Radiology department, we believe that successful outcomes are built on collaboration, precision, and patient-centered care.
