"Doctor, I used to walk to the temple and back. Now I have to sit down halfway."
I hear this almost every week in the OPD. The legs feel heavy, achy or numb after a short walk, and normal again after sitting for a few minutes. Many people blame age or weak knees. Often the cause is in the lower spine: the canal that carries the nerves has narrowed. We call it lumbar canal stenosis.
The family usually expects me to say "operate". Here is how I actually decide.
I do not advise surgery when:
• symptoms are mild and you manage daily work
• physiotherapy and medicines have not been tried properly
• only the MRI looks bad, but you walk comfortably
I discuss surgery when:
• walking distance keeps shrinking despite treatment
• leg weakness or numbness is increasing
• pain is limiting daily life
Emergency: loss of bladder or bowel control, or numbness around the groin. Go to hospital at once.
World Spine Day is on 16 October. Is someone at home walking less than they did last year? Tell me in the comments what stops them: pain, heaviness or fear of falling.
