Zieveβs Syndrome
Definition:
A triad of clinical findings seen in patients with alcoholic liver disease:
1. Hemolytic anemia
2. Jaundice (not solely due to hepatic failure, but also hemolysis)
3. Transient hyperlipidemia
Pathophysiology
β’ Exact mechanism not fully clear.
β’ Believed to result from:
β’ Alcohol-induced liver injury β impaired lipid metabolism.
β’ Abnormal lipoproteins destabilize RBC membranes β hemolysis.
β’ Alcohol and acetaldehyde directly toxic to RBC membranes.
Clinical Features
β’ Occurs in chronic alcoholics with acute alcoholic hepatitis or withdrawal.
β’ Symptoms: jaundice, fatigue, abdominal pain, nausea, vomiting.
β’ Lab findings:
β’ Hemolytic anemia (β reticulocyte count, indirect bilirubin, LDH; β haptoglobin).
β’ Hyperlipidemia (transient, can normalize within days to weeks).
β’ Abnormal liver function tests (AST > ALT, β GGT).
Management
β’ Alcohol cessation (mainstay).
β’ Supportive care: fluids, nutrition, transfusion if severe anemia.
β’ Lipid abnormalities usually resolve spontaneously once alcohol is stopped.
β’ Important to differentiate from other causes of jaundice and anemia in alcoholics (e.g., GI bleeding, cirrhosis, hepatitis).
