ACMOA Discusses Key Budget Proposals with the President

A meeting was held at the Presidential Secretariat with His Excellency President Anura Kumara Dissanayake, with the participation of Dr. Anil Jayantha Fernando, Deputy Minister of Finance and Planning , Dr. Harshana Suriyapperuma, Secretary to the Treasury and Ministry of Finance, Planning and Economic Development, other ministry officials, and leaders of the National Trade Union Centre (NTUC). ACMOA participated in the meeting as a constituent organisation of the NTUC.

As part of the ongoing discussions on the 2027 National Budget, ACMOA had the opportunity to discuss with His Excellency the President several proposals that had previously been formally submitted by ACMOA for consideration.

The discussion included improving quarters facilities for doctors, revision of the DAT allowance towards an appropriate transport allowance, and revision of income tax limits, particularly considering their impact on government medical officers.

The President gave a favourable response to these proposals, providing positive indications regarding the need to address these matters through the upcoming budgetary process.

ACMOA will continue to engage with the Government and relevant authorities to advocate for fair, practical and sustainable solutions for government medical officers while strengthening the public health service.

ACMOA SUBMITS PROPOSALS FOR THE 2027 NATIONAL BUDGET

The All-Ceylon Medical Officers Association (ACMOA) has submitted proposals to the Ministry of Finance for consideration in the preparation of the 2027 National Budget, focusing on the mobility, retention and welfare of medical officers and the sustainability of essential health services.

Our four key requests are:

🔹 Replace the existing DAT with a proper Transport Allowance through an amendment to Circular No. 22/99, with appropriate arrangements for Grade I & II Medical Officers and Medical Consultants.

🔹 Introduce a 4% Vehicle Purchase Loan Facility for eligible medical officers who have not previously benefited from a vehicle import concession.

🔹 Establish 1,000 Official Residential Units for Doctors, through a phased national programme, with priority given to rural, remote and underserved hospitals.

🔹 Review the restrictions on Overtime & Extra-Duty (OT/ED) payments, with consideration for lifting or partially relaxing the restrictions for essential health-sector services where additional duty is unavoidable.

ACMOA appreciates the Government's decision to increase the basic salaries of public-sector employees, including the further salary adjustment scheduled for January 2027.

At the same time, we believe that targeted measures addressing transport, accommodation and unavoidable additional duties are important to retain and effectively deploy medical officers across the country and to ensure the continuity of essential healthcare services.

ACMOA remains committed to engaging constructively and responsibly with the Government to develop financially sustainable and practical solutions within the prevailing fiscal framework.

LKR 8.84 MILLION FOR A SYSTEM THAT WAS ALREADY WORKING - WHO IS ACCOUNTABLE?

ACMOA has requested an independent investigation into the recent award of an LKR 8.84 million tender to Science Land Information Technology for a new Blood Bank Management System.

The existing system was developed by a Medical Officer and provided free of charge, and has been functioning successfully for approximately six years.
ACMOA fully accepts that a national system should not depend on one individual. It should be properly institutionalised, maintained, and developed under the responsibility of the Ministry/Health Informatics Unit (HIU).

But why spend LKR 8.84 million to replace a functioning system, instead of properly taking it over, maintaining it, and improving it?

This becomes even more concerning when ACMOA has repeatedly requested the full implementation and utilisation of existing Ministry-owned digital health systems. Some of these systems are modern, user-friendly and interoperable, yet remain underutilised, while there continues to be a strong push to develop and procure new software.

Why is there so little interest in using, improving and institutionalising systems that the Government already has, while there is continued enthusiasm for new software and new procurements costing millions of rupees?

ACMOA has also repeatedly raised concerns regarding HIU-related procurements, technical decisions and the conduct of certain officials.

The Ministry has already initiated investigations into previous concerns relating to the HIU.
Then what happened? Where are the findings? What action has been taken? And why do similar concerns continue to arise in subsequent digital-health procurements?

ACMOA is not making accusations without evidence. We are ensuring that such concerns are properly brought to the attention of the relevant authorities and independently examined.

We will continue to closely monitor decisions involving public funds, question questionable practices, and take appropriate action whenever the interests of the health service and the public are at stake.

We believe that transparency, accountability, and responsible use of public funds are not optional - they are essential to a functioning public health service.

ACMOA takes these matters seriously. We will continue to speak up, act responsibly, and stand for the public interest.

ACMOA–Ministry of Health Meeting: Key Issues Discussed

The All Ceylon Medical Officers Association (ACMOA) met with the Hon. Minister of Health, Hon. Deputy Minister of Health, and senior Ministry officials on 1 September 2026 to discuss several important issues affecting the health service and medical officers.

The meeting was attended by:
Hon. Dr. Nalinda Jayatissa – Minister of Health and Mass Media
Hon. Dr. Hansaka Wijemuni – Deputy Minister of Health
Dr. Anil Jasinghe – Secretary, Ministry of Health and Mass Media
Dr. Sampath Ranaweera – Deputy Director General (MSD)
Dr. Ayanthi Karunarathna – Director, Medical Services

Key matters discussed included:
🔹 Implementation of the Intermediate Grade for Medical Officers: Ministry officials indicated that the implementation process could be completed within six months, subject to the necessary administrative arrangements.
🔹 Implementation of shift duty systems to improve service delivery, staff utilisation and working conditions.
🔹 Accommodation facilities for Intern Medical Officers, with particular attention to providing safe and adequate accommodation during internship.
🔹 Continuing shortages of medicines and surgical consumables, with concerns regarding the availability of essential medicines and the resulting impact on the smooth functioning of hospitals and delivery of patient care.
🔹 Operational and supply-chain issues within the Medical Supplies Division (MSD) that require urgent attention to ensure a reliable and uninterrupted supply of medicines and surgical items.
🔹 Hospital bed management, with the Ministry agreeing to advise Hospital Directors on the implementation of proper bed management practices.

ACMOA appreciates the opportunity to raise these matters directly with the Hon. Minister and Ministry officials.

ACMOA remains committed to working alongside the Government and the Ministry of Health in strengthening the national health service, while constructively raising and advocating for solutions to issues affecting patients, healthcare workers and hospital services.

We hope that the discussions will lead to effective action on these longstanding issues in the best interests of patients, medical officers and the national health service.

Urgent Call for Institutional Responsibility on Intern Accommodation

With multiple medical batches being recruited simultaneously to clear backlogs, the total removal of accommodation provisions in recent posting letters (Ref: TCS/G/12/2026) leaves incoming Intern Medical Officers vulnerable.

While we recognize central constraints, hospital heads must take institutional responsibility for local lodging arrangements. ACMOA has formally written to the Hon. Minister of Health and Ministry Secretary requesting immediate intervention.

ACMOA has written to the Secretary of Health seeking urgent clarification regarding two major digital health procurements awarded under the HIQI Project.
Our concerns relate to:
▪ The award of the Common Registries project to a company that was previously the subject of serious performance concerns and discussions at parliamentary oversight forums.
▪ The award of the HHIMS-related project to a company whose experience in large-scale healthcare software development requires further clarification.
The Ministry of Health is currently investing billions of rupees in digital transformation initiatives. Such investments must be guided by transparency, proper governance, technical due diligence, and value for money.
Our intention is not to obstruct digital transformation, but to ensure that public and donor funds are utilized responsibly and that Sri Lanka's digital health future is built on robust, sustainable, and interoperable systems.
We have requested a detailed explanation of the evaluation and selection process and look forward to the Ministry's response.

Serious concerns have been raised over nearly Rs. 3 billion worth of digital health procurements under the HIQI Project through United Nations Office for Project Services.

A formal complaint has been submitted to the Investigation Division of the Ministry of Health requesting an urgent inquiry into possible duplication of existing Ministry-owned software systems, weak governance oversight, and inefficient use of valuable grant funds.

Sri Lanka already owns advanced digital health platforms and existing PACS systems in major hospitals. Public and donor funds must be used wisely, not for repeated procurement of parallel systems.

We call for:
✅ Immediate suspension of the procurements pending investigation
✅ Independent technical review with support from the Ministry of Digital Economy
✅ Full accountability if irregularities are identified

Digital health must move forward through integration, not fragmentation.
