In 2002, the Women’s Health Initiative (WHI) study made headlines: Hormone replacement therapy causes breast cancer!

And millions of women stopped taking hormones.

But here’s what the media got wrong, and why hormone therapy today may be less risky than you think.

1️⃣ The average participant was 63 years old, significantly older than most women starting hormone therapy today. Age matters because a woman’s baseline risk increases as she gets older. We cannot assume the results in women starting hormones in their 60s apply equally to women starting in their late 40s.

2️⃣ The hormones studied were NOT the same as the body-identical hormones we prescribe today. The study used conjugated equine estrogens (Premarin) and a synthetic progestin (Provera). That synthetic progestin may have contributed to the slightly increased breast cancer risk.

3️⃣ The estrogen-only arm actually had LOWER rates of breast cancer. Long-term follow-up found a reduction in breast cancer incidence among women taking estrogen alone after hysterectomy.

4️⃣ Headlines reported a 26% increased risk of breast cancer, but that was the RELATIVE risk in the combined hormone therapy group. The ABSOLUTE risk increase was only 8 additional cases per 10,000 women per year (0.08%). Naturally, news outlets emphasized the more alarming statistic.

The WHI is a textbook case of how a mathematically accurate relative risk can distort public perception when separated from absolute numbers. And the headlines didn’t tell the whole story.

The bottom line?

For many women, the benefits of hormone therapy outweigh the risks.

That doesn’t mean every woman can take hormone therapy, or that breast cancer risk is irrelevant. But it DOES mean women deserve informed, individualized conversations about the risks and benefits.
