When reimbursement rates go unchanged for years, the financial impact can be significant.

A physician-owned urology ambulatory surgery center in Mississippi came to PayrHealth with reimbursement rates that had not seen a meaningful update in nearly a decade. Four of its five largest commercial payors were reimbursing below 100% of CMS, while rising operating costs continued to put pressure on profitability.

Through market benchmarking, custom fee schedule design, and a coordinated contracting strategy between the ASC and affiliated clinic, the organization achieved:

✅ 80% rate improvement with its largest commercial payor, valued at $235,675 in year one
✅ 45.5% increase from a second payor
✅ Built-in 3% increases in years two and three
✅ Two contracts secured, with additional opportunities in progress

The biggest takeaway? Effective negotiations aren't just about asking for higher rates. They're about understanding your market, benchmarking appropriately, and building reimbursement models around the services you actually provide.

“The worst position in a negotiation is needing an answer faster than the payor does. Q4 is where you avoid that.”

— Scott Dewey, Chief Managed Care Officer at PayrHealth

Q4 is when next year's economics get set.

According to Scott, providers should focus on:

• Knowing which contracts are actually eligible for negotiation
• Prioritizing high-impact opportunities instead of chasing every agreement
• Using the leverage created by open enrollment periods
• Understanding policy changes that may be affecting reimbursement, even when contract rates haven't changed

Providers who benefit most from Q4 negotiations are typically the ones who start planning before the pressure is on.

What contract priorities are you evaluating before January 1?

“The terms you will operate under next year are being set right now, whether or not anyone at your organization is at the table.” — Armando Cardoso, CEO of PayrHealth

The end of the year has a way of surfacing what’s been sitting on the to-do list. For most provider organizations, several of those items are payor related, and they carry a deadline that’s closer than it appears. Most commercial agreements renew January 1, and many require 90 to 120 days written notice before renewal or termination. That places the decision point in October, not December.

What we’re seeing across our client base heading into 2027 is a widening gap between organizations treating Q4 as a planning quarter and those treating it as a closing quarter. The first group is pulling agreements, confirming notice dates, and benchmarking rates now. The second group will spend January reacting to terms that renewed without anyone deciding they should.

Any provider scheduled to start in Q1 2027 should already be in the enrollment process.

With payor enrollment timelines averaging 90 days or more, a January start date that begins credentialing in December produces a provider who’s seeing patients well before the claims are payable.

The same applies to a new location, an added service line, or an acquisition. The work can start before payor enrollment is done. The revenue can’t.

Worth confirming now, heading into Q1:

✅ Which providers start in Q1, and whether their applications are already submitted
✅ Whether new locations need their own enrollment or can be added to an existing agreement
✅ Whether licensure, supervision documentation, and CAQH profiles are current enough that applications won’t be returned

⏰ Last call — the Q4 edition of The PayrHealth Pulse goes live tomorrow.

This issue covers what to settle before January 1: the real deadline behind your renewal window, what Q1 hires need from credentialing right now, and how timely filing deadlines can quietly cost you before year-end.

We also sat down with Scott Dewey, our Chief Managed Care Officer, for his take on what providers should prioritize between now and January.

If you’re not on the list yet, now’s the time.

That's a wrap on Oncology Exchange Fall 2026!

This past weekend, our team had a great time connecting with oncology practice leaders, administrators, and industry partners. The event brought together valuable resources, networking, and thought leadership on the latest advances in oncology care, and the conversations we had reinforced how much payor strategy matters to a practice's ability to grow.

Thank you to everyone who stopped by to talk Payor Contracting & Strategy, Credentialing, and Revenue Cycle Management. We're already looking forward to continuing those conversations.

A special shoutout to Alyssia Bell and Scott Dewey for representing PayrHealth!

If we didn't get a chance to connect at the event, reach out. We'd love to hear what's on your practice's radar.

October is Breast Cancer Awareness Month.

At PayrHealth, we work with providers across specialties who are on the front lines of screening, diagnosis, and treatment access — and we know early detection depends on more than good intentions. It depends on patients being able to get in the door.
This month, we’re recognizing the clinicians, care teams, and administrative staff working to make screenings and follow-up care accessible to the patients who need them.

👉 However you’re able to, take a moment this month to schedule your own screening or encourage someone you love to schedule theirs.

The end of Q3 is when most practices find out whether the contracts they signed are performing the way they were supposed to.

"Signed rates and realized rates are not always the same number," says Kayla Schneider, SVP of Operations at PayrHealth, who oversees revenue cycle management. "Payment variances, downcoding patterns, and policy changes applied after signature all show up in the data before they show up in a conversation with the payor. By the time someone notices the shortfall in the monthly numbers, it has usually been happening for a while."

Her point is that contract performance is an operational question, not a legal one. The agreement sets the terms. What arrives in the remittance is what the practice actually operates on, and the gap between the two is only visible if someone is comparing them.

Reviewing performance now, while there is still time to act before renewal, is what separates a contract you manage from a contract that manages you.
