Bakersfield PACE by IIH Serving the Kern County community since January 2020.

Bakersfield PACE (Program of All-Inclusive Care for the Elderly) helps frail, low-income seniors live happy, healthy lives independently in their own homes as an alternative to nursing care.

At PACE, your care is handled by a whole team, one that meets regularly to talk about you.

It's called the Interdisciplinary Team, or IDT, and it typically includes:
🩺 Primary care physician
👩‍⚕️ Nurse practitioner
💊 Pharmacist
🩹 Registered nurse
🧠 Behavioral health specialist
🦵 Physical, occupational, and speech therapists
🥗 Dietitian
🤝 Social worker
🎨 Activities coordinator
🏠 Home care coordinator
🚐 Transportation team
💚 Personal care aides

Each brings a different lens. Together, they build one coordinated care plan around the participant and adjust it as life changes.
You're not one person's patient. You're the whole team's priority.

Staff spotlight: Roehl Caragao 🚴
Join us in congratulating Roehl Caragao, Health Broker at Fresno PACE by IIH, who recently completed the Eastern Sierra Double Century, a 200-mile bicycle race through the Sierra Nevada with more than 10,000 feet of climbing, much of it at elevations above 7,000 feet.
The route includes the famous June Lake Loop, a stretch of mountain road that winds past four alpine lakes beneath towering Sierra peaks. Riders call it the highlight of the ride, but earning that view takes months of training and the determination to keep pedaling for 200 miles in a single day.
At IIH, that spirit feels familiar. Going the distance is what we do every day for our participants: showing up with commitment, endurance, and care for the long haul. Roehl, thank you for inspiring us on and off the bike. We're proud to have you on the team!
