October is Menopause Awareness Month, and we want to talk about a symptom that rarely gets filed under menopause at all.

Frozen shoulder, or adhesive capsulitis, occurs most often between the ages of 40 and 60, and it affects women more than men. Patients describe it the same way almost every time: nothing happened, the shoulder just stopped working. Reaching for a seatbelt hurts, then sleeping on that side hurts, then the arm stops going overhead.

Estrogen plays a role in collagen structure and tissue elasticity, which is part of why the shoulder capsule can thicken and contract during the menopause transition. Blood sugar and thyroid function affect it, too, and both commonly shift in the same years.

We assess it as one problem rather than two. Diagnostic ultrasound shows us the capsule and the surrounding tissue, hormone and metabolic panels show us the environment that tissue is sitting in, and treatment can include osteopathic manipulative therapy, guided injection, and addressing whatever the labs turn up.

Tai-Chi and Qi-Gong kicks off tomorrow evening, and there is still room in the fall series.

Nick Kusturic, Lic. AP, a certified Tai-Chi and Qi-Gong instructor, leads the hour. Tai chi is a sequence of gentle exercises and stretches where one posture flows into the next without stopping, which is why it often gets described as meditation in motion. Qi Gong works on the same principle from a different angle, using breath and movement together to steady energy and focus. Both build balance and joint control, and both calm a nervous system that has been running hot.

A practical note as we head into October.

Patients tell us often that they have wanted a full workup for a year and keep putting it off. If you want it done before the year ends, the timing matters more than most people expect.

Our comprehensive evaluation covers hormone panels, advanced metabolic markers, inflammatory assessment, and gut health. The sequence runs like this: an intake visit where we take your full history, lab orders, two to three weeks for specialty panels to return, and a follow-up appointment long enough to walk through what the results show and what we would do about it.

That is four to six weeks from first call to a plan you can act on. An early October start puts you on the other side of it before the holidays. 🧪

One of the harder conversations in pain medicine happens when the imaging looks normal and the patient still hurts.

Pain that persists past tissue healing often involves central sensitization. The nervous system has amplified and maintained a signal that no longer corresponds to active damage. Patients are frequently told at this point that there is nothing left to try, or that the problem is psychological. Neither is accurate.

At the Marcus Institute, the nervous system is part of the pain workup. Osteopathic manipulative therapy addresses the structural restrictions and guarding patterns that keep feeding the signal. Acupuncture helps shift an overactive sympathetic response. For patients whose fight-or-flight state has become the primary driver, a stellate ganglion block can interrupt that loop and create room for other treatment to work.

We also assess sleep, inflammatory load, and hormone patterns, all of which change pain threshold. 🧘‍♂️
