Starting 7 days before your child’s appointment, log into the Patient Portal to update demographics and insurance information and complete annual consents—all on your own time! 💻💚

Complete Pre-Check In before you arrive, and the only paperwork left at your visit will be any needed screening forms. ✔️

Same Day Access Schedule

Outpatient mental health and substance use assessments are available on a walk-in basis during our same day access hours at our Bradford, Randolph and Wilder offices.

No Insurance? We offer a sliding fee scale option, applications available at your same day access appointment.
