Here’s how I scan the thyroid 🦋

I start with a transverse midline view showing the right lobe, left lobe, and isthmus, with and without color Doppler. Then, I image and measure the isthmus.

Next, I scan the right lobe in transverse, obtain my measurements, and sweep from superior to inferior. In sagittal, I obtain images with and without color Doppler, measure the lobe, and sweep from lateral to medial.

Then, I repeat the same sequence for the left lobe!

Basic Pelvic Ultrasound Protocol

Here’s a quick breakdown of a basic pelvic ultrasound protocol:

• Uterus — Sagittal
Sweep right ➡️ left through the entire uterus and obtain your sagittal measurements.

• Uterus — Transverse
Sweep superior ➡️ inferior and obtain your transverse measurements.

• Endometrium
Evaluate the endometrium, obtain its measurement, and document any abnormal findings.

• Adnexa
Evaluate the right and left adnexa.

• Ovaries
Identify both ovaries, obtain your measurements, evaluate with color, and obtain Doppler.

• Cervix
Evaluate and measure the cervix if it’s part of your protocol.

• ⁠• Cul-de-sac
Check for any free fluid.

If you identify any pathology or abnormal finding in the uterus, endometrium, ovaries, adnexa, or surrounding structures, document and measure it appropriately.

Remember, protocols can vary depending on your facility.

Sometimes you’re so busy working toward what’s next that you forget to look back at how far you’ve already come🥹🤍

Born in Brooklyn and raised between Brooklyn & Puerto Rico, I saw and experienced things growing up that shaped me in ways most people will never know.

Today, I’m the first college graduate on both sides of my family, a Registered Diagnostic Medical Sonographer, and growing two businesses while continuing to dream even bigger🎓✨

This patient has Hashimoto’s disease, and her thyroid ultrasound demonstrated multiple nodules.

Ultrasound allows us to evaluate the thyroid’s size and appearance and carefully document each nodule, including its location and measurements. These findings are then reviewed by a radiologist, who provides the official report.
