We are excited to announce that our value-based care collaboration with Aetna, a CVS Health Company, has expanded to South Carolina. Now, eligible Aetna Medicare Advantage members across the state will be able to benefit from our comprehensive, home-based approach to treating patients with multiple chronic conditions. While Monogram Health does not replace the primary care relationships for patients and is designed to be an additional resource and extension of the patient’s current care team, Monogram takes a comprehensive and personalized approach to a person’s health, simultaneously evaluating and treating the whole person and all chronic conditions that are present – such as diabetes, hypertension, chronic kidney disease, heart failure, depression, COPD, and other metabolic disorders.

Since 2023, Monogram Health has successfully collaborated with Aetna, a CVS Health Company, to care for more than 55,000 attributed patients across Florida, Iowa, New Jersey, Pennsylvania, Texas, West Virginia - and now South Carolina.

Amid hurricane season, Ms. Rangel faced disrupted access to her treatment. At Monogram, care doesn’t stop when a storm starts. Monogram’s Emergency Preparedness Program (EPP) is dedicated to providing critical support during these challenging times. In addition to ensuring our patients have every medical need met in advance of, during, and after a natural disaster - we also provide support/access to food, transportation, financial planning/recovery, and more.

This week, Monogram Health attended the Health Innovation Summit and hosted a panel on “Integrated, Person-Centered Care and Population Health” led by Monogram’s Chief Medical Officer, Dr. Shaminder Gupta, Chief Cardiologist, Dr. Susan Bell, and Chief Pulmonologist, Dr. Lakshman Swamy.

In addition to analyzing the true value of multispecialty treatment and care for complex patients, they also highlighted the clinical and financial outcomes associated with this next-generation, in-home, cross-disciplinary care, as well as the technology and AI required to support clinicians and patients.

This independent Accreditation validates Monogram's person-centered, in-home clinical model against the industry's most rigorous population health management benchmarks. When a dangerous potassium spike hit, Ms. Jenkins didn't visit the ER, she called Monogram Health FIRST. After a discussion with a Monogram physician over the phone, she received details on how to care for herself in the moment and the flare-ups she was experiencing (i.e., revised diet, medication review, and daily activities to refrain from). The very next morning, a specialist with the right medication in hand was at her doorstep to review what they discussed the day prior. Ms. Jenkins never had to panic, never had to go to the emergency room, and never needed to leave the comfort of her home.
