You told me the plan is "finally doing something" because you slept through the night. Write down which night.
Here's why that one sentence matters more to me than any form you'll fill out.
A scheduled check-in asks you to grade yourself on cue. But the night you slept through, the morning the socks went on without you thinking about it, nobody prompted that. Your own body flagged the change before I ever asked. That unasked-for observation is the richest data in the room, because it marks a real shift in the environment around the nerve, the inflammation and the blood supply it depends on.
So I ask you to pin it to a date. Not a vague "a bit better." A specific night. Because the volunteered good moment, dated, tells me more about what's actually changing than a box checked on a page.
